粗颗粒物质(PM10-2.5)对缺血性卒中影响的证据有限且不一致。近日,血管疾病领域权威杂志Stroke上发表了一篇研究文章,研究人员旨在评估PM10-2.5暴露对中国缺血性卒中住院的急性影响。

研究人员对2014年1月至2016年12月期间中国每日PM10-2.5浓度与每日缺血性卒中住院率之间的关联进行了全国时间序列分析。从城镇职工基本医疗保险数据库中确定了缺血性卒中的住院率。该医疗保险包含了来自1.8亿受益人的数据。研究人员应用城市特定泊松回归来评估PM10-2.5与每日缺血性卒中入院之间的关联,并将城市特定效应估计与随机效应荟萃分析相结合,并进一步评估暴露-效应关系曲线和潜在效应修正。

研究人员在172个中国城市确定了200多万名缺血性卒中住院患者。PM10-2.5浓度每增加10μg/m3(滞后第0天)与缺血性卒中住院率增加0.91%(95%CI为0.73-1.10)相关。调整PM2.5后,该关联仍然显著(百分比变化为0.96%; 95%CI为0.75-1.18)。暴露-效应关系近似为线性,在较低水平(<200μg/m3)具有中等反应,在较高水平具有较陡的反应。在PM10-2.5浓度较低、温度较高或相对湿度较高的城市,这种关联更为密切。

由此可见,这项全国范围的研究提供了PM10-2.5暴露与增加的缺血性卒中住院率之间短期关联的有力证据,并支持这一假设,即该关联因城市特征而异。
