Diabetes Care:1型糖尿病儿童和青少年确诊后尽早启动连续血糖监测能改善诊断后3年的HbA1c结果

本研究发现在1型糖尿病确诊后6个月内开始使用CGM的儿童和青少年,在确诊3年后的HbA1c水平更低,该结果强调了在1型糖尿病确诊后尽早使用CGM的重要性。

连续血糖监测(continuous glucose monitorCGM)对1型糖尿病(type 1 diabetesT1D)儿童和青少年患者有许多已知益处,且随机对照试验和真实世界研究均表明,CGM使用与改善血糖结果之间存在关联。来自单个中心的研究数据表明,在诊断后一年或更长时间内,早期启动使用CGM对血糖结果产生积极影响。本研究利用T1DX-QIT1D Exchange Quality Improvement Collaborative)中25个儿科中心的数据,旨在评估1型糖尿病儿童和青少年患者CGM启动时间与诊断后3HbA1c之间的关联。

本研究数据来源于T1DX-QI的电子健康记录数据库,涵盖了2019年和2020年在25家儿科中心确诊为T1D18岁及以下的儿童和青少年。在确诊后3年内对CGM的启动时间和血糖结果进行了跟踪。利用局部估计散点图平滑法评估CGM启动时间与HbA1c随时间变化的关系,并使用逻辑回归模型调整潜在的混杂因素。
