Neurology:血清NFL可区分帕金森病与非典型帕金森病

近日,神经病学领域权威取杂志Neurology上发表了一篇研究文章,该研究旨在探讨血清神经丝轻链(NFL)对帕金森病明显症状的诊断价值,但具体诊断尚不确定。

研究人员采集了具有明显帕金森病症状但仍未确诊的患者血清样本。经过3年的随访,明确了帕金森病(PD)和非典型帕金森综合征(APD)的临床诊断,并在最长12年后再次更新,以便获得更长的随访数据。在患有PD(n=55)和APD(n=29,多系统萎缩=22,进行性核上性麻痹=7)和53个非神经对照的患者中通过单分子阵列定量了血清NFL水平。

血清NFL水平升高,APD组(平均值为23±10.3 ng/L)与PD(平均值为10.4±4.9 ng/L)和对照组(平均值为11.5±6.5 ng/L,p<0.0001)相比,准确度上升至91%(灵敏度=86%,特异性=85%)。血清NFL与所有组个体CSF中NFL水平(r=0.72,p<0.0001)和PD个体的年龄(r=0.78,p<0.0001)和对照(r=0.66,p<0.0001)强烈相关。在该队列中,患APD的概率为76%(阳性预测值),患PD的概率为92%(阴性预测值)。

由此可见,与PD相比,APD患者血清NFL水平显著升高,并且可以高精度地区分APD与PD。即使在临床症状尚未确定的阶段,血清NFL也可能是识别APD的有用临床生物标志物。
