    本文对从痛风患者和健康对照组纵向收集的307份肠道微生物组进行了大规模的微基因组学分析。利用发现和验证队列,我们试图调查痛风肠道微生物组的分类和功能特征,它与痛风相关临床参数(如血清尿酸(SUA)和炎症)的关系,它对治疗干预的反应,以及痛风相关微生物组特征与更广泛的自身免疫和代谢紊乱的关系。

     综上所述,我们在与痛风相关的肠道微生物群中确定了分类和功能特征,并根据我们的结果提出了肠道微生物如何影响痛风发展的假设模型。类杆菌和普罗维菌素的富集可能导致痛风肠杆菌中6-酰基链类脂A的生物合成发生改变,从而对宿主免疫刺激和内毒素耐受性产生负面影响,从而导致4-或5个酰基链的类脂A的生物合成发生改变。同时,肠杆菌属物种的枯竭可能导致尿酸降解功能障碍,从而导致全身尿酸蓄积和痛风炎症的增加。其他微生物功能,如SCFA的产生和鞭毛组装,可能有助于维持健康的肠道微环境,它们在痛风中的耗尽可能通过改变宿主受体而导致局部和全身炎症增加。结果显示痛风患者肠道菌群失调与尿酸升高和全身炎症有关,随着时间的推移,降尿酸和抗炎药物干预可能会部分恢复。未来需要对更大的纵向队列进行多组学研究,结合动物模型实验,以验证该发现,以更好地理解痛风肠道微生物区系的潜在机制。
