肌肉发生胰岛素抵抗是2型糖尿病的典型特征之一,并且在糖尿病发生之前很长时间内就已经存在。普遍认为体育活动能够改善对血糖的控制,但是其中的隐藏机制还没有得到完全了解。体育锻炼的过程中会伴随着肌肉氧气水平下降,进而导致转录因子HIF-1a得到诱导表达。HIF-1a是许多基因表达所需要的一个关键转录因子,因此可能在骨骼肌功能和代谢中发挥重要作用。

最近来自德国的科学家Jürgen Eckel带领研究团队通过研究发现HIF-1a对骨骼肌的葡萄糖代谢和胰岛素作用发挥有重要作用。相关研究结果发表在国际学术期刊Diabetes上。

他们通过全基因组基因表达分析发现RAB20和TXNIP是骨骼肌中受运动/HIF-1a调控表达的两个新基因。RAB20的缺失会损伤胰岛素刺激的葡萄糖摄取,这种情况在人和小鼠的骨骼肌中都会出现,进一步的研究表明RAB20缺失以后会阻断葡萄糖转运蛋白GLUT4向细胞表面的转定位,导致骨骼肌细胞对葡萄糖的摄取受到损伤。除此之外,TXNIP是一个已知能够对胰岛素作用产生负调控的调控因子,而运动/HIF-1a能够下调TXNIP的表达解除其对胰岛素作用的抑制。
