Circulation:生活方式干预对2型糖尿病患者心衰风险的影响

2性糖尿病(T2DM)与心衰(HF)风险更高有关。但生活方式的干预和心肺适能(CRF)和体重指数的改变对心衰风险的影响尚不明确。

本研究纳入Look AHEAD试验中无HF的受试者。比较强化生活方式干预组和糖尿病支持和教育组发生HF的风险。

共5109位受试者,,中位随访超过12.4年,强化生活方式干预组和糖尿病支持和教育组之间的HF风险无明显差异(HR 0.96)。在最校准的Cox模型中,中度适合组和高度适合组的HF风险分别降低了39%和62%。在HF亚组中,在对传统心血管危险因素和心肌梗死的间隔发生率进行校整后,基线CRF与心衰发生率及射血分数降低的风险无显著相关性。相比之下,中度适合组发生保留射血分数的HF的风险降低了40%,而高度适合组降低了77%。基础体重指数也与发生HF、保留射血分数的HF、或调整CRF和传统心血管危险因素后射血分数降低的HF的风险无关。

在重复CRF评估的受试者(3902人)中,4年随访期间CRF的改善和体重的减轻与HF风险的降低显著相关(CRF每增加10%的HR 0.90;BMI每减少10%的HR 0.80)。

在Look AHEAD试验的2型糖尿病患者中,强化生活方式干预与HF风险变化无关。基础CRF越高、CRF持续改善和减重均与HF风险降低有关。
