　　近日，一项发表于2012年9月《美国胃肠病杂志》的研究指出，与血管造影相比，不明原因急性显性消化道出血病人使用即时胶囊内镜有更高的诊断率以及更好的长期预后。 　　胶囊内镜和血管造影曾被推荐作为不明原因急性显性消化道出血患者（OGIB）的首选检查，但在此类患者中没有研究在直接对这两种检查进行比较。研究者对比了不明原因急性显性消化道出血患者随机使用胶囊内镜和血管造影检查的诊断率和长期随访结果。

　　胶囊内镜和血管造影曾被推荐作为不明原因急性显性消化道出血患者（OGIB）的首选检查，但在此类患者中没有研究在直接对这两种检查进行比较。研究者对比了不明原因急性显性消化道出血患者随机使用胶囊内镜和血管造影检查的诊断率和长期随访结果。

　　连续有急性黑便或者便血但不能由上下消化道内镜确诊的患者.随机接受小肠胶囊内镜或血管造影检查。对所有患者进行长达5年再出血和贫血症的监测。主要终点为的指定检查的诊断率，次要终点包括再出血、再次输血、因出血或贫血再次入院、死亡。

　　随机化60名不明原因急性显性消化道出血患者。平均随访时间为48.5个月。发现即时胶囊内镜诊断率明显高于血管造影（53.3% vs. 20.0%, P=0.016）。血管造影组和胶囊内镜组再出血的累积风险分别为33.3%和16.7%（P=0.10, 时序检验/对数秩检验），两组之间长期预后如再次输血、因出血再次入院治疗或死亡并无明显差异。
