JAHA:基于血浆代谢物的心脏手术相关急性肾损伤预测

高分辨率代谢组学对于开发新的生物标志物来说效应足够强大。3种代谢物的血浆水平可用于早期识别CSA-AKI患者。

心脏手术相关的急性肾损伤(CSA-AKI)是心脏手术后常见的术后并发症。目前,尚无可靠的方法对住院患者的CSA-AKI进行早期预测。

近日,心血管疾病领域权威杂志JAHA上发表了一篇研究文章,该研究旨在开发并评估基于代谢组学的生物标志物在CSA-AKI患者中的诊断用途。

该研究共招募了214名个体(122名急性肾损伤[AKI]患者,92名无AKI患者作为对照组)。研究人员使用非靶向和靶向代谢组学方法通过液相色谱串联质谱法分析了受试者的血浆样品。在AKI猪模型中研究了选定代谢物的时间依赖性效应。研究人员使用多种机器学习算法来识别与CSA-AKI呈正相关的血浆代谢物。

研究人员发现心脏手术后24小时内采集的血浆样本中代谢组学分析有助于区分AKI患者和无AKI的对照者。葡萄糖酸、富马酸和假尿苷在AKI患者中显著上调。研究人员发现使用选定的临床参数和代谢物构建的随机森林模型具有出色的判别能力(曲线下面积为0.939;95%CI为0.879-0.998)。在AKI猪模型中,3种可区分的代谢物血浆水平以时间依赖性方式增加(R2为0.480-0.945)。然后在验证队列中确认了该AKI预测模型的效应(曲线下面积为0.972;95%CI为0.947-0.996)。在验证队列中早期AKI患者亚组中进行测试时,预测模型的效应仍然稳健(曲线下面积为0.943;95%CI为0.883-1.000)。

由此可见,高分辨率代谢组学对于开发新的生物标志物来说效应足够强大。3种代谢物的血浆水平可用于早期识别CSA-AKI患者。
