JAMA:射血分数保留心力衰竭患者应当如何进行体育锻炼?

高强度间歇耐力运动以及连续中等强度耐力运动均可改善射血分数保留心力衰竭患者的峰耗氧量,但与基于指南的体育锻炼相比,改善效果未达到预先设定的最小临床重要差异

峰值耗氧量(peak VO2)是评价心力衰竭患者病情程度和判断预后的客观指标,研究证实适量的耐力运动有助于改善射血分数保留心力衰竭患者(HFpEF)峰耗氧量,但目前尚不清楚运动模式在其中发挥的作用。近日研究人员评估了3种耐力运动方式--高强度间歇运动、强度适中的连续运动和基于指南的体育锻炼对HFpEF患者峰值VO2的影响。
 
研究在德国、比利时和挪威的5个医疗中心开展,2014年7月至2018年9月期间,180名慢性,病情稳定的HFpEF患者参与,患者随机分为高强度间歇运动组(每周3次,每次38min高强度耐力训练)、中等强度连续运动组(每周5次,每次40min中等强度耐力训练)和对照(基于指南的体育运动,每周1次),各组均包含60名参与者,持续12个月。研究的主要终点为3个月后峰值VO2变化,次要终点包括3个月和12个月后心肺适应度、舒张功能和利钠肽指标变化。
 
参与者平均年龄70岁,女性占67%,92%的参与者完成3个月评估,86%的参与者完成12个月评估。3个月时,与基线相比,高强度间歇运动组、中等强度连续运动组和对照组峰值VO2变化为1.1 mL/kg/min、1.6 mL/kg/min和-0.6 mL/kg/min,但12个月时组间差异无统计学意义。组间舒张功能或利钠肽指标无明显变化。4名高强度间歇运动组患者出现急性冠脉综合征(7%),而中等强度连续运动组和对照组分别有3例(5%)和5例(8%)。
