Neurology:7T MRI发现部分脑微出血源自静脉

该研究是首次在体内系统地研究大脑小静脉与MBs之间的空间关系的人类神经成像研究。该研究的发现建立了脑MB与静脉脉管系统之间的联系,并指出了静脉可能对CSVD尤其是对CAA的贡献。

脑微出血(Cerebral microhemorrhage, MBs)是小的低信号性圆形病变,在T2 -加权磁共振成像(MRI)和磁敏感加权成像(SWI)上均可见。散发性CSVD最常见的形式是脑淀粉样血管病(CAA),主要累及软脑膜和皮质小动脉,以及高血压动脉病(HA)的深穿动脉。组织病理学相关研究表明,MRI上大多数MBs是急性、亚急性或慢性小灶性病变,由小血管渗漏或破裂导致完整红细胞或含铁血黄素累积。CSVD主要病变为小动脉和小动脉,因此提示MBs主要来源于动脉树。然而,在CAA和AD动物模型和人类CAA中均观察到皮层和软脑膜静脉淀粉样蛋白-ß (Aß)积累。此外,在老年、CSVD和临床前AD模型发现了静脉胶原变性的证据。因此,小静脉在由CSVD相关静脉病理引起的MBs发展中的作用也是可以想象的。到目前为止,病理和体内研究还没有研究静脉对MBs形成的贡献。

最近Neurology发表文章,分析了一组CSVD患者和老年对照组的亚毫米分辨率的基于磁敏感定量成像技术(QSM)的静脉图,首次在体内检测小静脉和MBs之间的关系。研究静脉在MB形成过程中的可能的作用,评估了MBs在体内是否存在与静脉的直接关系。

该研究进行了7-Tesla(7T)MRI扫描,并在磁敏感定量成像(QSM)上对MB进行了计数。亚毫米分辨率的基于QSM的静脉图允许通过与静脉的直接空间连接来识别MB。

共有51人(平均年龄[SD] 70.5 [8.6]岁,女性37%)参加了该研究,其中20人患有CSVD(脑淀粉样血管病[CAA] 脑叶型MBs [n = 8],高血压性动脉病[HA]深部MB 型[n = 5],或脑叶和深部MB混合型[n = 7],年龄为72.4 [6.1]岁,女性占30%),另有31名健康对照者(69.4 [9.9]岁,女性42%)。该研究统计了总共96处有静脉连接的MB,占7T QSM上所有检测到的MB的14%。大多数静脉型MB(86%,n = 83)位于脑叶,所有在皮质层。CAA患者的静脉型MB/总MB占比(19%)最高,HA患者为9%,混合型为18%,对照组为5%)。

这一高分辨率的7T神经成像研究研究了在CSVD患者队列中是否可以检测到小静脉和MBs之间的空间关系。QSM可用于显示大脑MBs与静脉以及两者是否有直接联系。与静脉有连接病变占所有MBs的14%。该研究支持MBs可能不完全源自动脉的观点,静脉可能在MBs和(可能的)CSVD发病机制中发挥重要的作用但尚未深入研究。例如,静脉胶原病导致小大脑静脉壁增厚。这可能导致腔内压力增加和血脑屏障(BBB)渗漏,这是CSVD和(可能)MB启动的关键机制。静脉胶原沉积进一步与WMH和腔隙的发生有关。与此相一致的是,在CSVD中,用于评估深髓静脉病理改变的半定量视觉评分也与WMH体积和多发性MBs的存在独立相关。静脉和MB之间的直接关系也曾在7t MRI研究中被观察到。然而,到目前为止,还没有专门的研究来区分动脉和静脉来源的MBs。
