Radiology:构建0-6岁ADC变化图谱

该研究对0~6岁儿童磁共振扩散加权成像的ADC发育模式进行了量化。这项工作提供了有关大脑早期发育的知识,并可能有助于对成熟异常的检测。

目前关于儿童早期发育阶段的正常扩散加权成像模式的信息有限。缺乏描述从出生到儿童早期(1-7岁)表观扩散系数(ADC)值变化的规范数据。而这个阶段是大脑变化最快的最关键时间点。大脑ADC值的变化对于理解大脑成熟的正常区域变异很重要。对于许多疾病来说,这一发育期是早期诊断大脑疾病的关键时间点。由于临床放射科医生在很大程度上依赖扩散加权成像来区分异常,了解和量化正常发育的模式是很重要的。

该研究对某医院2006~2013年收集的201例健康足月儿童(男108例,年龄0~6岁)的年龄ADC图谱进行分析,以量化ADC变化轨迹、ADC变化率、ADC成熟年龄和大脑半球成熟年龄不对称性等四个指标。通过拟合两项指数模型,在全脑、分割脑区和体素水平对上述指标进行了量化。用Bonferroni校正t检验评估两侧半球ADC成熟年龄的不对称性。

初始ADC值为内囊后肢(平均ADC左半球1.18×103μm2/s;右半球1.17×103μm2/s)、内囊前肢(左侧1.11×103μm2/s;右侧1.09×103μm2/s)、小脑蚓部(1.26×103μm2/s)、丘脑(左侧1.17×103μm2/s;右侧1.15×103μm2/s)、基底节(左侧1.26×103μm2/s;右侧1.23×103μm2/s),提示为脑发育的较早期胎儿阶段。ADC的成熟在1.3到2.4岁之间完成,不同脑区时间不同。小脑蚓部和左侧丘脑成熟最早(1.3岁)。额叶外侧灰质成熟最晚(右侧2.3岁,左侧2.4岁)。左半球ADC成熟较早的白质区域包括额叶(左2.16±0.29岁;右2.19±0.31岁)、颞叶(左1.93±0.22岁;右1.99±0.22岁)和顶叶(左1.92±0.30岁;右2.03±0.28岁)。右半球ADC成熟较早的白质区域包括丘脑(左1.63±0.32岁;右1.45±0.33岁)、基底节(左1.79±0.31岁;右1.70±0.37岁)和海马(左1.93±0.34岁;右1.78±0.33岁)。

体素水平分析ADC下降率
