探索头条:Cell子刊连发两文:胰岛干细胞移植有望终结1型糖尿病

两项研究均表明了PSC 衍生的胰腺祖细胞在皮下植入时具有存活、移植、分化和成熟为人类胰岛样细胞的能力。

糖尿病是一种慢性疾病,影响全球超过 4.6 亿人。据估计,中国20岁以上的成年人中有超过1.139亿糖尿病患者和4.934亿处于糖尿病前期者。也就是说,糖尿病的发展态势正像“流行性感冒”一样越来越严重,糖尿病后备军也在大幅度增加。
 
1型糖尿病(T1D)是一种代谢紊乱综合征,这种疾病主要是由于通过免疫介导胰腺β细胞的破坏而导致,患者经常伴有多种严重的并发症,有时甚至会危及生命。虽然人工胰岛素治疗可以控制血糖浓度,减少病发病,但达不到治愈的效果。
 
在过去的 20 年里,胰岛移植,即恢复内部调节的胰岛素和胰高血糖素的产生为治疗难以控制的 T1D 患者提供了一种安全有效的治疗选择。然而,胰岛细胞疗法需要大量替代供应的胰岛素生成细胞。除了对猪胰岛的兴趣、胰岛素分泌细胞系的开发和原位细胞重编程策略外,人类多能干细胞的使用在成为大量生产胰岛素细胞的可行临床选择方面取得了巨大进展。
 
12月2日,Cell Stem Cell 和 Cell Reports Medicine 期刊同时发表了关于胰岛素移植的研究论文。
 

Cell Stem Cell》期刊发表了一篇题为“Implanted pluripotent stem-cell-derivedpancreatic endoderm cells secrete glucose-responsive C-peptide in patients withtype 1 diabetes”的研究论文。文中首次提供证据表明,干细胞衍生的 PEC-01s(胰腺内胚层细胞,在植入后26周内存活并成为对葡萄糖敏感、分泌胰岛素的细胞)可以在1型糖尿病患者体内成熟为葡萄糖反应性、产生胰岛素的成熟 β 细胞
 
在长达一年的随访中,患者的胰岛素需求减少了20%,在目标血糖范围内花费的时间增加了13%。总之,植入物耐受性良好,无严重移植物相关不良事件。
 
早在2014年,ViaCyte对它们的候选产品PEC-01推出了开放性试验以调查其安全性、耐受性和功效。据报道,前19名患者对该产品的耐受性良好并且几乎没有并发症。2017 年又启动了一项后续试验,以研究封装在非免疫保护设备中PEC-01s 的安全性、耐受性和 C 肽产生。初步结果表明:PEC-01s 在植入后 26 周内存活并成熟为葡萄糖反应性胰岛素分泌细胞,并且患者在目标血糖范围内花费了更多时间。
 

而在《Cell Reports Medicine》期刊上发表的“Insulinexpression and C-peptide in type 1 diabetes subjects implanted with stemcell-derived pancreatic endoderm cells in an encapsulation device”的研究又进一步阐释了有63%的PEC-01细胞在植入体内3-12个月后被观察到植入和胰岛素表达。
 
此外,功能性胰岛素分泌细胞在植入后大约 6-9 个月的时间内逐渐积累。
 
总之,两项研究均表明了PSC 衍生的胰腺祖细胞在皮下植入时具有存活、移植、分化和成熟为人类胰岛样细胞的能力。尽管研究中都存在一定的局限性,很多问题都没有进一步研究和细化,但是我们看到了对于治疗1型糖尿病里程碑式的跨越和突破。期待以干细胞为主的糖尿病治疗方法能在不久的将来为患者带来福音。
