Neurology:脑膜转移瘤患者脑脊液循环上皮肿瘤细胞分析

近日,神经病学领域权威取杂志Neurology上发表了一篇研究文章,该研究的主要目的是明确可疑软脑膜转移瘤(LM)患者CSF中通过上皮细胞粘附分子(EpCAM)免疫流式细胞术进行循环肿瘤细胞(CTC)分析的敏感性和特异性。次要目标是探讨非小细胞肺癌NSCLC)中原发性肿瘤、血浆、无细胞CSF(cfCSF)和从CSF分离出的CTC中致病突变的分布。

研究人员在一项前瞻性研究中对81例临床怀疑为LM但MRI不确定的患者进行了CTC分析和CSF细胞学检查的性能测试。在NSCLC亚群中,研究人员通过数字液滴PCR(ddPCR)分析了所选致病突变的循环肿瘤(ct)DNA。

在0.9 CTC/mL的最佳临界值下,CTC分析的灵敏度为94%(95%置信区间[CI] 80–99),特异性为100%(95%CI 91-100)。细胞学检查的敏感性为76%(95%CI 58-89)。23例NSCLC患者中有12例发生了表皮生长因子受体(EGFR)突变。所有5位经测试的LM患者在cfCSF中均表现出主要的EGFR致病基因突变。也可以在从CSF分离的CTC中检测到致病突变。

由此可见,脑脊液中CTC对LM的诊断具有较高的敏感性。ddPCR可以确定EGFR突变的NSCLC和LM患者的cfCSF和来自CSF的CTC中的EGFR突变。
