Eur Radiol:颅外静脉异常(是多发性硬化患者真实病理学表现还是正常的解剖变异?!)

MS病变多见于脑室旁白质静脉区,SWI之父就曾试图利用SWI来探究MS病变的分布机制是否与铁沉积相关,或者是与静脉狭窄相关。研究人员对MS与颈静脉的相关性提出质疑,并进行了相关研究,并将结果发表在Eur Radiol上。

目的:利用对比增强磁共振静脉造影技术评估不伴有多发性硬化症(MS)患者的颅外静脉解剖结构,并评估各种静脉异常的发生率,如不对称性和狭窄。

材料与方法:我们前瞻性地招募了100例非多发硬化的患者,年龄在18-60岁之间,行对比增强MRI扫描。 他们在3T扫描仪上进行额外的从颅底至纵隔CE-MRV扫描。 排除标准包括先前的颈部辐射,颈部手术,颈部/纵隔肿块或明显的心脏或肺部疾病。 两名神经放射学家独立评估并记录颈静脉及突出的侧支静脉的不对称性及狭窄。

结果:75%的受试者发现颈内静脉不对称。两名观察员在颈内静脉狭窄方面达成一致。大多数的狭窄位于颈内静脉的上段。椎静脉和颅外突出的侧枝静脉,特别是颈外静脉的不对称性,并不罕见。

结论:对于非多发硬化患者,颈内静脉及颈部突出的侧支静脉的不对称性及狭窄是非常常见的。这些发现与先前的报道形成鲜明对比,之前认为,除了在多发硬化患者中,其他情况,侧支静脉的狭窄和不对称非常罕见。
