近年来,独立的临床站点(如输液中心)正变得越来越普遍。这些站点需要及时的血液学分析。在这里,我们比较了目前可用的分析配置的性能和成本,特别关注一个替代方案,即使用一个最小的血液学分析仪和一个数字成像设备,从而实现远程监督和解释。本研究的目的是确定低级实验室是否可以节省开支,同时以建议的替代方案取代常用的配置,从而实现日常功能。

为评估提出替代的性能配置,过使用标准的回归分析,95% CIs和真值表对血细胞计数与自动差异产生的Sysmex XE5000(完整的血细胞计数参考方法)细胞计数的Sysmex poch – 100i CellaVision DM96 preclassified,DM96重新分类差异(微分参考方法) 进行比较。在加州大学圣地亚哥卫生系统进行现场血液学分析的远程诊所中观察到的人员配置、检测量和涂片生产率进行财务建模。

研究结果发现,差异血计数参数显示XE5000R2 > 0.95的预先分类DM96之间具有极好的相关性。对于母细胞/异常细胞、未成熟的粒细胞和有核红细胞,DM96XE5000具有更高的敏感性和相似的特异性。成本模型显示,通过DM96所促进的结果远程监控,可以降低人员成本,从而降低与更传统的分析配置相比的操作成本。

研究表明,数码影像仪器配以便宜的血液分析仪可提供与复杂血液分析仪相似的结果,并可让专家远程检视血液检验结果,大大节省成本。
