细胞色素 P450 环氧合酶 2J2 (CYP2J2) 将花生四烯酸代谢为环氧二十碳三烯酸 (EET),其对心血管系统发挥抗炎、抗凋亡、促增殖和抗氧化作用。同样,其在肺动脉高压中具有保护作用。

肺缺血再灌注损伤 (LIRI) 是心肺手术中常见的并发症,如肺移植、肺动脉重建和体外循环。随着人口老龄化,继发于心肺疾病的肺动脉高压(PAH)的发病率逐年增加,因此许多接受心胸手术的患者可能患有 PAH。对于这些患者,如何减少围手术期LIRI和术后并发症已成为一个重要的问题。然而,对于降低 PAH 患者肺缺血再灌注过程中的 LIRI 没有达成共识,并且很少有研究在动物模型中对 PAH 和 LIRI 的影响进行了研究。因此,迫切需要对 LIRI 联合 PAH 进行深入研究,以全面了解 PAH 与 LIRI 的影响,探索 PAH 与 LIRI 的防治策略。

细胞色素 P450 环氧合酶 2J2 (CYP2J2) 将花生四烯酸代谢为环氧二十碳三烯酸 (EET),其对血管系统发挥抗炎、抗凋亡、促增殖和抗氧化作用。然而,CYP2J2 和 EETs 在肺动脉高压 (PAH) 和肺缺血再灌注损伤 (LIRI) 中的作用仍不清楚。因此,我们研究的目的是确定 CYP2J2 和 EETs 是否对受 PAH 影响的 LIRI 发挥保护作用,并通过体内和体外实验阐明它们的作用机制。

研究方法:

通过重组腺相关病毒(rAAV)将CYP2J2基因转染大鼠肺组织,以增加血清和肺组织中EETs的含量。通过腹腔注射野百合碱(50 mg/kg)4周,然后夹住左肺门1小时,再灌注2小时,构建LIRI大鼠PAH模型。此外,我们建立了TNF-α联合缺氧/复氧(缺氧8小时和复氧16小时)的人肺动脉内皮细胞(HPAECs)细胞模型,以确定外源性EETs的作用和机制。

研究结果:

CYP2J2基因转染增加大鼠肺组织CYP2J2蛋白和血浆EETs含量;Fig. 2CYP2J2 过表达降低大鼠的 mPAP 和 RVSP,并减弱了组织病理学的变化;Fig. 4

CYP2J2过表达显着降低了与LIRI肺损伤相关的炎症反应、氧化应激和细胞凋亡。此外,外源性 EETs 抑制炎症反应,减少细胞内活性氧 (ROS) 的产生,并抑制肿瘤坏死因子α (TNF-α) 联合 HPAECs 缺氧-复氧模型中的细胞凋亡。我们进一步的研究表明,CYP2J2 过表达和 EETs 的抗炎作用可能是由 PPARγ 的激活介导的。抗凋亡作用可能是由 PI3K/AKT 通路介导的。

研究结论:

CYP2J2 过表达和外源性 EET 通过体内和体外的抗炎、抗氧化应激和抗细胞凋亡减少了 LIRI 的 PAH。抗炎作用可能由 PPARγ 的激活介导,抗凋亡作用可能由 PI3K/AKT 途径介导。CYP2J2 过表达和 EETs 给药可以成为预防和治疗 LIRI 的 PAH 的新策略。
