子宫内膜癌:预测淋巴结转移模型

挪威卑尔根大学Berg等报告的一项研究证明,数据驱动的预测模型将蛋白质标志物添加到子宫内膜样子宫内膜癌(EEC)的临床信息中,具有显著改善术前淋巴结转移(LNM)患者的确诊潜力。(Br J Cancer. 2020 doi10.1038/s41416-020-0745-6.

EEC中,当前的临床算法无法准确预测伴随LNM的患者,这就导致了治疗不足和过度治疗。该研究旨在开发将蛋白质数据与临床信息相结合的模型,以识别需要更积极手术(包括淋巴结清扫术)的患者。

使用反相蛋白阵列为399例患者生成了蛋白表达谱。使用训练集构建了三个广义线性模型,包括蛋白质和临床信息(模型1)、还包括磁共振成像信息(模型2)和仅有蛋白质信息(模型3),并在独立的验证集中进行了测试。利用来自肿瘤的基因表达数据确认测试。

用曲线下面积0.72-0.89 和细胞周期蛋白D1预测LNM;纤连蛋白和疾病等级被确定为重要的标志物。纤连蛋白和细胞周期蛋白D1的高水平与不良的生存率有关(P=0.018),并与肿瘤浸性标志物有关。FN1CCND1信使RNA的双重上调与癌症的侵袭和间质表型有关。
