Hepatology:影响1/4的成年人——脂肪肝的风险远不止肝癌!

非酒精性脂肪肝(NAFLD,俗称脂肪肝)是最常见的慢性肝脏疾病,约有25%的成年人深受影响。

非酒精性脂肪肝(NAFLD,俗称脂肪肝)是最常见的慢性肝脏疾病,约有25%的成年人深受影响。NAFLD,包括非酒精性脂肪肝(NAFL),以及由其演变的非酒精性脂肪性肝炎(NASH)、脂肪性肝纤维化、肝硬化等。

可能很多中年人甚至年轻人的体检报告单上都曾出现过“轻度脂肪肝”这样的警告,却认为“无关紧要”而没有引起重视。

而在去年发表在胃肠道顶尖杂志GUT上的研究——由瑞典卡罗林斯卡学院和美国马萨诸塞州总医院的研究人员在研究了瑞典51年的队列研究数据后报道称,即使是轻度脂肪肝也会增加71%死亡风险,且风险与脂肪肝的严重程度成正比。

值得注意的是,随着社会经济发展和人民生活水平的提高,近年来我国脂肪肝发病率骤升。

2019年,武汉大学前基础医学院院长李红良教授发表在《肝脏病学》(Hepatology)杂志上的meta分析显示,近10年来,中国的脂肪肝患者从18%跃升到29.2%,尤其是中年人、男性、西北地区、台湾地区、人均生产总值大于10万元的地区以及维吾尔族和回族为“重灾区”。

大家都知道NAFLD与肝癌的风险密切相关,但其它癌症情况如何呢?最近,来自瑞典卡罗林斯卡研究所和美国麻省总医院的肝脏病学专家再次对NAFLD患者癌症风险进行了全面的研究,结果再次发表在Hepatology杂志上。

研究人员对1966-2016年瑞典所有具有组织学定义的NAFLD的成年人进行了一项基于人群的队列研究(N=8892)。NAFLD的定义来自于提交给瑞典所有28个病理中心的前瞻性记录的肝脏组织病理学,并分为NAFL、非纤维化非酒精性脂肪性肝炎(NASH)、肝纤维化和肝硬化。

NAFLD患者与无NAFLD的普通人群对照组按≤1:5的比例根据年龄、性别等进行个体匹配(N=39907)。使用Cox比例危险模型,计算了多变量调整后的危险比(aHRs)和95%置信区间(CIs)。

在13.8年的中位随访时间里,共有NAFLD患者组合对照组分别发生1691例6733例癌症事件。与对照组相比,NAFLD患者的总体癌症发病率显著增加27%(10.9 vs. 13.8/1000人年;差异=2.9/1000人年;aHR=1.27),且其作用主要由HCC驱动(差异=1.1/1000人年;aHR,=17.08)。换句话说,NAFLD患者肝癌风险增加16倍!

具体根据NAFL、NASH、肝纤维化和肝硬化分层发现,所有患者的HCC发生率也增加(每1000人年增加0.8、1.2、2.3、6.2,且在合并糖尿病时更为明显。与对照组相比,上述不同阶段的NAFLD患者分别与肝细胞癌风险升高61%、145%和395%相关。

同时,NAFLD还会增加胰腺癌、肾癌、膀胱癌和黑色素瘤的发病率。而与对照组相比,NAFLD对肝外癌的贡献不大。

综上,在脂肪肝的所有阶段,均会明显增加肝细胞癌以及整体癌症发病风险。

近期,中国人民解放军总医院联合青岛大学公共卫生学院的研究人员在Foods发表一项研究,表明相比于猪油、大豆油,调和油更有益于肝功能标志物,或有助于降低健康人群非酒精性脂肪肝发生风险。

该研究显示,与单独食用猪油或豆油相比,食用调和油(50%猪油+50%豆油),更有益于肝功能标志物,AST和ALT的降低更为显著,或有助于预防健康人群的非酒精性脂肪肝。
