Eur Radiol: SNAP序列在验证动脉斑块内出血方面的价值!

斑块内出血(IPH)是颈动脉易损动脉粥样硬化斑块的关键特征之一,IPH的存在会加速斑块进展并增加缺血性脑血管事件发生的风险。因此,对IPH的定性和定量评估将有助于临床进行脑血管事件的风险分层,并为进一步优化患者的管理提供更精确的信息。

已有多项研究证明,MP-RAGE序列在检测经组织学验证的颈动脉IPH中具有最高的敏感性和准确性。最近,可同时提供颈动脉MRA和IPH评价的SNAP序列越爱越受到临床的关注。然而,SNAP序列在定性、定量评估颈动脉IPH的的综合效能尚未得到组织学方面的证实。

近日,发表在European Radiology杂志的一项研究从组织学上验证SNAP成像与MP-RAGE成像在表征颈动脉IPH中的性能。

方法:对35例计划行颈动脉内膜切除术的患有颈动脉粥样硬化疾病(症状性50-70%狭窄或> 70%狭窄)的患者行包括SNAP和MP-RAGE序列的3.0-T颈动脉MR成像。 在SNAP和MP-RAGE图像上分别评估了IPH的存在和面积。 同时还以组织学为基础对IPH的存在和面积进行了评估。使用Cohen kappa、Spearman相关性和Bland-Altman分析来确定和定量IPH,从而确定SNAP 及MP-RAGE与组织学之间的一致性。

 

    在所有35例患者中(平均年龄:63.1±8.8岁,男性27例),共有128例切片符合分析条件。SNAP和MP-RAGE诊断IPH的准确率、敏感性、特异性、阳性预测值和阴性预测值分别为86.7%、85%、89.6%、93.2%和78.2%,MP-RAGE诊断IPH的准确率、敏感性、特异性、阳性预测值和阴性预测值分别为76.6%、75%、79.2%、85.7%和65.5%。在IPH的定性中,SNAP和MP-RAGE与组织学的kappa值分别为0.725和0.520。在测量IPH面积时,SNAP与组织学的相关性(r = 0.805, p < 0.001)高于MP-RAGE与组织学的相关性(r = 0.637, p < 0.001)。Bland-Altman分析显示,在测量IPH面积时,SNAP的偏差(1.4 mm2, 95% CI: - 0.016 ~ 2.883)小于MP-RAGE的偏差(1.7 mm2, 95% CI: - 0.039 ~ 3.430)。
