我们对肾素-血管紧张素-醛固酮系统阻滞稳定的2型糖尿病早期糖尿病肾病患者进行了双盲、随机、交叉试验的事后分析,患者白蛋白与肌酐比值在100~3500 mg/g之间,eGFR≥45 ml/min/1.73 m2,糖化血红蛋白≥7.2%且<11.4%。患者在连续6周的研究期间随机服用达帕格列净10 mg/d或安慰剂,间隔6周洗胃。我们研究了对循环磷酸盐、钙、甲状旁腺激素(PTH)、成纤维细胞生长因子23(FGF23)、25-羟基维生素D(25[OH]D)和1,25-二羟基维生素D(1,25[OH]2D)水平的影响。

 

研究共分析了31名患者(年龄62岁;23%女性)。结果显示,与安慰剂相比,dapagliflozin使血清磷酸盐增加9%(95%置信区间,4%~15%;P=0.002),PTH增加16%(3%~30%;P=0.01),FGF23增加19%(0.3%~42%;P=0.05),血清1,25(OH)2D下降-12%(-25%~4%;P=0.12)。钙和25(OH)D不受影响。我们发现,在达帕格列津治疗期间,磷酸平衡的标志物的变化与eGFR或24小时白蛋白排泄的变化之间没有相关性。

 

综上所述,该研究结果表明,Dapagliflozin可增加血清磷酸盐、血浆PTH和FGF23。这种影响与eGFR或24小时白蛋白排泄的伴随变化无关。
