由此可见,下呼吸道的细菌负荷可预测特发性肺纤维化的疾病进展,并且与放射疾病的程度无关。

特发性肺纤维化患者下呼吸道细菌负荷的增加会使疾病进展和死亡的风险增加。但是,尚不清楚这种增加的细菌负荷是否直接影响纤维化的进展或仅反映了潜在疾病程度或严重程度。

近日,呼吸疾病领域权威杂志Eur Respir J上发表了一篇研究文章,研究人员前瞻性地招募了193例接受了支气管镜检查并接受了多学科诊断的特发性肺纤维化患者。通过16S rRNA基因qPCR对支气管肺泡灌洗液中总细菌负荷进行定量。影像学检查结果由两名阅片者独立评估,他们为影像学变化的程度、严重性进行定量评分,并评估了这些特征与细菌负荷之间的关系。

细菌负荷增加与疾病进展显著相关(风险比为2.1; 95%置信区间为1.287-3.474;p=0.0028)。多元逐步回归表明细菌负担与影像学特征或疾病程度之间没有关系。当考虑患有确定或可能的常规间质性肺炎患者时,两组之间的细菌负荷没有差异。尽管胸膜间质纤维弹性增生和临床感染之间有假定的联系,但胸膜间质纤维弹性增生的存在或程度与细菌负荷之间没有关系。

由此可见,下呼吸道的细菌负荷可预测特发性肺纤维化的疾病进展,并且与放射疾病的程度无关。
