虽然α1-抗胰蛋白酶(α1AT)在自然情况下不能抑制酶接触系统,但人为给α1AT加一个突变(M358R;α1AT-Pittsburgh)即可将其改造成一种强大的广谱酶抑制剂。α1AT-Pittsburgh不仅能阻断接触系统,还能阻断凝血酶和活化蛋白C (APC),使其不适合用作治疗性接触系统阻断剂。

研究人员尝试改变α1AT-Pittsburgh的反应中心环(AIRS/S)来克服上述障碍。两种α1AT突变体(SMTR/S和SLLR/S)均可强烈地抑制血浆激肽释放酶激活FXII和胞浆素。α1AT-SMTR/S不再抑制凝血酶,但对APC还残留一点抑制作用。相反,α1AT-SLLR/S还可稍微抑制凝血酶,但对APC没有抑制作用了。

P1’位点上的附加修饰(S→V)消除了这两种突变体对凝血酶和APC残留的抑制作用,同时保留了它们作为接触系统抑制剂的特性。此外,两种突变体均可阻断角叉菜胶诱导的小鼠急性组织水肿。最后,α1AT-SLLR/V,本研究最突出的候选突变体,还可抑制结肠炎小鼠模型的肠道上皮细胞漏。

总而言之,本研究证明了α1AT再修饰可明显改变其抑制行为,可用于治疗接触系统介导的血栓形成和炎症。
