原发性侧索硬化症(PLS)是一种罕见的上运动神经元(UMNs)退化性疾病,临床表现为进行性痉挛。疾病的进展比肌萎缩性侧索硬化症(ALS)慢。PLS的诊断不确定性由于缺乏特定的神经化学生物标记而变得复杂。尽管ALS中的脑脊液(CSF)神经丝水平明显升高,使其成为反映轴突变性的公认的ALS生物标志物,作者测量了PLS,ALS和非神经退行性神经疾病患者的脑脊液中磷酸化的神经丝重链(pNFH)和Chit1。每种生物标记物可区分PLS和ALS。

方法:PLS的10例患者(5例男性,5例女性),患者的脑脊液储存在生物库中。 患者满足PLS的诊断标准。 ALS患者(n = 28; 16名男性,12名女性)符合经修订的El Escorial诊断标准,从研究中的生物库随机选择了ALS和NCs患者。腰穿后2小时内将离心的CSF样品保存在-80°C下。 所有患者均检测到pNFH; 在10例PLS患者中有10例,28例ALS患者中的20例以及30例NC中有21例测量了Chit1。 使用ROC曲线评估每个单个生物标记和组合生物标记的诊断性能。

结果:该研究的主要发现是pNFH和Chit1在三个诊断类别中均不同(PLS,ALC,NCs)。关于pNFH和Chit1的含量,PLS相对于ALS和NCs之间显示出中等水平。,这可能反映了与ALS相比,PLS中皮质脊髓轴突的变性较慢,同样,PLS中Chit1的降低可能反映了较小程度的小胶质细胞神经炎症,这可能与轴突丢失有关,这是由两个生物标记之间的相关性反应出来的。从临床的角度来看,除了确认已知的ALS中pNFH和Chit1的脑脊液水平明显升高以及pNFH在该疾病中的预后价值外,作者的研究还表明pNFH和Chit1两者在诊断PLS方面均具有潜在的实用性。
