据悉,血小板可辅助机体的先天免疫反应抵抗炎症,虽然目前缺乏该功能在包括疟疾在内的任何人类疾病中的直接证据。近日,Blood杂志上发表一篇文章,Steven Kho等人采用显微镜和流氏细胞术对各种疟原虫(恶性疟原虫、P.vivax、三日疟原虫或诺氏疟原虫)感染的患者的血小板-红细胞之间的相互作用进行探究。

研究人员在疟疾流行区收集到376例血样。在本研究涉及的所有疟疾类型中均可观察到血小板可与红细胞内的疟原虫直接结合、并将其杀死,特别是成虫,在P.vivax疟疾患者中最明显。在血流中,血小板优先与感染的红细胞结合。

对红细胞内的寄生虫分析显示血小板相关的寄生虫杀死频繁发生,特点是红细胞内血小板因子4累积和寄生虫细胞核的脱氧核苷酸转移酶末端的dUTP镍端标记(PF4+TUNEL+寄生虫)。

PF4+TUNEL+寄生虫与系统性血小板激活无关。患者血小板计数、感染的红细胞-血小板复合物和血小板相关的寄生虫杀伤与患者的寄生虫负荷量负相关。上述关联,加上在不同患者和疟原虫类型中观察到的血小板相关寄生虫杀伤的频率,提示血小板可能可控制5-60%的循环寄生虫的生长。在疟疾患者中,血小板-红细胞复合物会消耗机体的大部分血小板,因此可能促进疟疾血小板减少症的发生。研究人员还通过诺氏疟原虫证明了寄生虫杀伤受血小板因子4介导。

总而言之,本研究表明血小板直接参与人类疟疾的疟原虫感染的固有控制。
