JASN:血液透析可导致老年患者脑血流量急剧下降

因此,传统的血液透析能显著降低老年患者整体和区域CBF。重复的透析引起的CBF减少可能是通过血液透析诱导的脑缺血损伤的机制之一。

启动血液透析与认知功能加速下降以及脑血管意外和白质病变的发病率增加有关。研究者推测透析的重复循环应激可导致缺血性脑损伤,但其机制尚不清楚。近日,肾脏病领域权威杂志Journal of the American Society of Nephrology上发表一篇研究文章,研究人员了分析了常规血液透析对通过[15O]H2O正电子发射断层扫描–CT(PET-CT)测得的脑血流(CBF)的急性效应。

单次血液透析期间,进行了三次[15O]H2O PET-CT扫描:在血液透析开始前、开始后早期和结束时。研究人员采用线性混合模型分析了血液透析中整体和区域脑血流量变化。十二名年龄≥65岁的患者(女性五例,男性七例)完成了研究,透析时间的中位数为46个月。

平均动脉血压从透析前的101±11mmHg下降至透析结束时的93±17mmHg。从血液透析开始前到结束时,整体CBF下降了10%±15%,从平均34.5降至30.5ml/100g/min(差异为-4.1ml/100g/min;95%可信区间为-7.3至-0.9 ml/100g/min;P = 0.03)。一例患者CBF下降(20%)时出现症状。区域CBF在所有感兴趣范围内下降,包括额叶、顶叶、颞叶和枕叶、小脑和丘脑。较高的鼓室温度、超滤量、超滤速率、pH值与低CBF密切相关。

因此,传统的血液透析能显著降低老年患者整体和区域CBF。重复的透析引起的CBF减少可能是通过血液透析诱导的脑缺血损伤的机制之一。
