NEJM:Voxelotor治疗镰状细胞性贫血

研究发现,voxelotor显著提高镰状细胞性贫血患者血红蛋白水平,降低溶血标记物

去氧镰刀血红蛋白(HBS)聚合驱动镰状细胞病的病理生理学,直接抑制HBS聚合有可能改善疾病。近日研究人员考察了HBS聚合抑制剂Voxelotor的疗效。

在本次III期临床研究中,研究人员比较了两种剂量水平的voxelotor(1500mg和900mg,每日一次口服)与安慰剂治疗镰状细胞疾病的疗效和安全性。主要终点是血红蛋白反应的参与者的百分比,其定义为第24周较基线增加超过1.0g/dL。

共有274名受试者,大多数参与者患有镰状细胞性贫血(纯合子血红蛋白S或血红蛋白Sβ0-地中海贫血),大约三分之二的人在基线时接受了羟基脲治疗。在意向性治疗分析中,1500 mg voxelotor治疗组(51%)受试者的血红蛋白反应率明显高于安慰剂组(7%)。24周内,所有接受voxelotor治疗人群的贫血风险降低。在第24周,与安慰剂组相比,1500 mg voxelotor治疗组的间接胆红素水平和网织红细胞百分比明显降低。组间不良事件率相近,1500 mgvoxelotor治疗组26%的受试者发生至少3级不良事件,900 mg voxelotor组为23%,安慰剂组26%。大多数不良事件与试验药物或安慰剂无关。
