ARD：慢性皮肤红斑狼疮病人的B细胞都一样吗？

B细胞对系统性红斑狼疮（SLE）的作用是明确的，但在慢性皮肤红斑狼疮（CCLE）中的作用目前还不清楚。因而，这篇研究比较SLE，CCLE和两者兼有患者的B细胞与自身抗体谱。

   B细胞对系统性红斑狼疮（SLE）的作用是明确的，但在慢性皮肤红斑狼疮（CCLE）中的作用目前还不清楚。因而，这篇研究比较SLE，CCLE和两者兼有患者的B细胞与自身抗体谱。

   采用流式细胞术比较健康对照、CCLE无SLE（CCLE+/SLE-）、SLE有CCLE（SLE+/CCLE+）或SLE无CCLE（SLE+/CCLE-）患者共四组的B细胞亚群。血清用ELISA法测量自身反应性9G4+，抗双链DNA，抗染色体和抗RNA抗体，用荧光素酶免疫沉淀法测抗RNA结合蛋白（RBP）。

    CCLE+/SLE-患者与SLE患者具有相同的B细胞亚群异常虽然程度较SLE患者轻：未转换记忆B细胞减少和效应B细胞增加。类似地，CCLE+/SLE-患者与SLE患者9G4+ IgG自身抗体水平均升高，虽然CCLE+/SLE-患者的抗核酸抗体和抗RBP抗体水平较低。CCLE+/SLE-患者可以被分成类似SLE的B细胞组成（血清检测抗体水平更高、更易于有播散性皮肤损伤）和正常B细胞组成。

结论：

     CCLE的B细胞平衡被打破、部分B细胞耐受性被破坏。CCLE在免疫学上是异质性的，包含一部分像SLE的B细胞组成，临床表现也与自身抗体水平升高和更广泛的皮肤损伤相关。这提示原发性CCLE患者中具有像SLE患者B细胞组成改变的可能在日后会发展成SLE。检测CCLE的B细胞组成也可能筛选出从B细胞靶向治疗中获益的患者。
