纵隔淋巴结肿大除少数为原发于纵隔的淋巴瘤或霍奇金病外,大多继发于其他脏器疾病。纵隔淋巴结肿大可无特殊症状。但肿大的淋巴结可压迫食管产生吞咽困难 ,也可压迫气管产生呼吸困难。如压迫上腔静脉可产生上腔静脉阻塞综合征,患者有头、面、上胸部肿胀。出现纵隔压迫症状以恶性肿瘤为多,少数结核性纵隔淋巴结肿大形成纵隔肉芽肿或广泛纤维化时也可引起上腔静脉阻塞综合征。本研究旨在评价体素内不相干运动(IVIM)扩散模型

本研究共纳入了35例发生MLN增大的患者,并行1.5T MRI扫描。扩散加权扫描采用8个b值进行扫描。在单指数模型得到IVIM参数包括D、D*、f和表观扩散系数(ADC)。91个结节的结果由病理学结果(n=90)和放射学结果(n=1)确定。利用ROC曲线分析诊断效能。

恶性组D、ADC和f值要明显低于良性组(p<0.001),而D*值间并无统计学差异(p=0.281)。在ROC分析中,与ADC和其他单独IVIM参数相比,结合D和f值获得了最大的曲线下面积(0.953),相应最佳特异性和诊断准确率分别为92.9%、90.1%。

研究结果表明,与单独ADC值相比,结合IVIM参数能够增加鉴别良恶性MLN的价值。
