幽门螺杆菌(Helicobacter pylori, H. pylori)是引起胃炎、胃溃疡及胃癌等消化系统疾病的重要病原菌。近年来,随着抗生素的广泛使用,幽门螺杆菌的抗药性问题日益突出,严重影响根除治疗的成功率。本研究系统鉴定了与抗药性相关的关键基因突变,构建了高准确性的抗药性基因突变目录,首次实现基因型与表型抗药性的高度一致性评估,揭示了全球不同地区耐药突变的流行分布,为个体化治疗和分子诊断提供了有力支持。 研究人员收集了来自全球50个国家的1011株H. pylori菌株全基因组序列(HpGP项目),并对其中419株进行了克拉霉素和左氧氟沙星的表型药敏测试。使用Etest法测定最小抑菌浓度(MIC),耐药判定标准分别为克拉霉素MIC>0.5 mg/L,左氧氟沙星MIC>1 mg/L。利用基因组数据,提取23S rRNA和gyrA基因序列,识别与耐药相关的关键突变,包括23S rRNA的A2142G、A2142C、A2143G和gyrA的A88V/P、N87K/I、D91G/N/Y突变。通过统计学方法评估基因型与表型抗药性的敏感性、特异性及预测价值,并分析区域分布及菌株
