背景

方法和发现
研究人员将所有数据库中关于二甲双胍与孕妇的相关文章纳入本项研究,主要观察结局是胎儿,婴儿和儿童期生长的参数,包括体重,身高,体重指数,和身体成分。共有28项研究(n = 3976名参与者)符合资格标准,并纳入荟萃分析。 19项研究(n = 3723名新生儿)报告了新生儿生长的指标。与胰岛素治疗的母亲的新生儿相比,使用二甲双胍治疗的孕妇的新生儿出生体重较低(平均差异-107.7 g,95%CI -182.3至-32.7,I 2= 83%,p = 0.005)以及较低的重量指数(平均差异-0.13 kg / m 3),95%CI -0.26至0.00,I 2 = 0%,p = 0.04)。与胰岛素治疗的孕妇相比,二甲双胍治疗的巨大儿的几率(优势比[OR] 0.59,95%CI 0.46-0.77,p < 0.001)和孕龄大(OR 0.78,95%CI 0.62-0.99,p = 0.04)较低。两组间新生儿身高或小于胎龄的发生率无差异。有限的证据(1项研究)表明,与胰岛素治疗的母亲相比,二甲双胍治疗的儿童的肥胖指数(腹部[ p =0.02]和内脏[ p = 0.03]脂肪体积)可能更高。

结论
孕期内暴露于二甲双胍用于治疗的孕妇,新生儿明显小于母亲在怀孕期间接受胰岛素治疗的新生儿。尽管平均出生体重较低,但暴露于二甲双胍的儿童出生后会经历了加速的出生后生长。
