较高的NT-proBNP和NT-proANP水平与较低的dCA效率相关。这些结果表明循环NP与脑自动调节之间存在关联,可能是将NP与不良脑血管预后联系在一起的机制。

利钠肽(NP)升高与不良的脑血管疾病有关,包括卒中、脑小血管疾病和痴呆。但是,这些关联的机制尚未明确。近日,心血管疾病领域权威杂志JAHA上发表了一篇研究文章,在这项研究中,研究人员评估了脑利钠肽前体N末端(NT-proBNP)和心房利钠肽前体N末端(NT-proANP)与脑血管功能的关系。

研究人员纳入了年轻人冠状动脉风险发展(CARDIA)队列中的154名参与者(平均年龄为56±4岁),使用电化学发光免疫测定法和酶联免疫测定法分别从第25年评估时采集的血液样本中测量了NT-proBNP和NT-proANP水平。动态颅脑自动调节(dCA)是在第30年评估中通过经颅多普勒超声检查确定的,研究人员使用自发性血压和流速振荡的传递函数分析(相位和增益),其中较低的相位和较高的增益反映了较低的大脑自动调节效率。研究人员使用了多变量线性回归模型,该模型针对人口统计资料、血管危险因素以及肾脏和心脏病史进行了调整。

第25年较高的NT-proBNP水平与较低的相位(β=-5.30)、较低的相位幅度(-10.05至-0.54)和较高的增益(β[95%CI]=0.06[0.004-0.12] cm/s mmHg)相关。类似地,较高的NT-proANP水平与较低的相位有关(β=-9.08)和较低的相位幅度(-16.46至-1.70)相关 。

由此可见,较高的NT-proBNP和NT-proANP水平与较低的dCA效率相关。这些结果表明循环NP与脑自动调节之间存在关联,可能是将NP与不良脑血管预后联系在一起的机制。
