CELL:克罗恩病患者肠道中的“爬行脂肪”元凶竟是肠道菌群

脂肪组织的显著可塑性被认为是为了储存多余的能量或为饥荒时期做储备而存在的。已有的研究显示,脂肪并不是一个被动的组织,而是一个由多种细胞类型组成的复杂环境,它们能够对许多生理过程作出反应并产生影响。其中一个最明显的例子是肠系膜脂肪组织(MAT)迁移到克罗恩病(CD)患者的肠道炎症病灶,导致所谓的 "爬行脂肪"。

爬行脂肪(CrF)是CD的一种肠外表现,常出现在纤维化、严重并发症的患者中,表现为增生性MAT,它会特异性地扩张并包裹在肠道炎症部位,主要在小肠出现,最常见的是回肠。

CrF未见于溃疡性结肠炎(UC),即炎症性肠病(IBDs)的另一种形式。自1932年首次发表观察结果以来,CrF一直是个谜。CrF在视觉上也是引人注目的,因为CD肠道中的炎症是斑块状的,而不是连续的。因此,CrF也被观察到是不连续的,呈手指状,“抓住”炎症(涉及)肠段,毗邻正常(未涉及)MAT。

CrF本身和底层肠组织往往是严重纤维化的,因此,CrF和肠管狭窄常常同时发生。值得注意的是,脂肪经常会侵犯到肠肌层。然而,CrF纤维化作为肠纤维化的驱动因素或减轻因素之间的关系尚未被探讨,尽管它们在患者组织呈现亲密的空间关系和一致的表现。

鉴于炎症诱导的屏障功能障碍是CD病变的显著特征,研究人员推测这些病变处的CrF现象是对微生物从炎症回肠转移到周围MAT做出的反应。越来越多的证据表明,肠道通透性是慢性肠道炎症性疾病的一个组成部分,屏障功能受损是IBDs中公认的病理特征之一。这反过来又会使其他身体部位暴露于肠道微生物群和/或其代谢产物中,引发免疫反应和肠道周围脂肪的生理变化。

在最近CELL杂志发表的一项研究中,研究人员探讨CD中的微生物易位是否可以作为CrF发展的核心线索。利用从手术切除中收集的人类MAT,研究人员发现了粘膜相关肠道细菌的一个子集,在CD回肠手术切除的CrF中持续转位并保持活力,并确定了Clostridium innocuum菌株作为这个联合体的标志。Clostridium innocuum在粘膜和脂肪分离物之间存在菌株变异,表明其偏好富含脂质的环境。

跨疾病和健康组织对照的MAT转录组谱,结合在患者衍生的原代细胞中的体外验证,促使研究人员将CrF定性为主要是纤维化和免疫原性的组织,其用于微生物监测的细胞表型显著上调。给gnotobiotic小鼠灌胃C.innocuum的CrF衍生菌株,可以导致MAT扩张和C.innocuum的恢复,因此,C.innocuum转位到MAT的能力被前瞻性地证实了。

此外,研究人员在人类和小鼠中发现的纤维化和脂肪生成表型与肥胖症中描述的内脏脂肪表型相似。这表明,其在CD中观察到的微生物驱动的MAT扩张可能与更广泛的脂肪扩张的发病机制有关。
