Liver Int：吸烟增加了罹患非酒精性脂肪性肝病的危险性

尿可替宁证实的当前吸烟和自我报告的吸烟是NAFLD的独立危险因素。进一步的纵向研究需要更清楚地阐明吸烟对NAFLD发展的影响。

吸烟与非酒精性脂肪性肝病(NAFLD)之间的关系一直存在争议。大多数相关研究结果都依赖于问卷调查。本研究的目的是使用一种客观的烟草暴露生物标志物(尿可替宁)和自我报告的问卷，来阐明吸烟状态与NAFLD之间的关系。

在2011年4月至2015年12月间进行了160862例无症状的腹部超声和检测尿液可替宁水平。当前吸烟者被定义为尿可替宁水平≥50 ng/mL。

研究人群，平均年龄为36.1岁，男性的比例为51.7%。据报道，自报和由尿可替宁证实的吸烟者的比例分别为17.6%和17.7%。在调整了混杂因素后，自我报告的当前吸烟会增加罹患NAFLD的风险(调整优势比[AOR]：1.10；95%置信区间[CI]：1.06-1.14)。此外，在目前的吸烟者中，NAFLD的风险随着尿可替宁含量的增加而增加(而非吸烟者相比，<10和≥10包年的吸烟者；优势比分别为1.04和1.11；95% CI分别为1.01-1.08和1.05-1.16)。尿可替宁证实的当前吸烟也与NAFLD风险的增加有关(AOR， 1.10；95%可信区间，1.06 - 1.14)。

尿可替宁证实的当前吸烟和自我报告的吸烟是NAFLD的独立危险因素。进一步的纵向研究需要更清楚地阐明吸烟对NAFLD发展的影响。
