要看懂这项研究,我们得先来说说癌症的转移。从统计数字上看,癌症造成的死亡病例,有90%要归因于癌症的转移。这不难理解。癌症转移意味着癌细胞在身体的其他部位扎根生长,且这些细胞往往已经对现有的疗法产生了耐受。因此在许多医生的眼中,癌症转移,就意味着无药可治。

许多科学家们想要解决这一难题,而他们的出发点各不相同。有的科学家想要找到癌细胞耐药的原因,杀死那些发生转移的癌细胞。另一些科学家则想从源头制止癌细胞的转移,巴塞尔大学的Nicola Aceto教授就是其中之一。

关于癌细胞为什么会发生转移,人们有过很多猜测。早在19世纪中叶,澳大利亚的病理学家Thomas Ashworth博士就推测,循环肿瘤细胞(CTC)可能是转移的根源。这些细胞从最初的肿瘤上脱落下来,会进入到血液循环中,游遍全身。一旦找到合适的位置,它们就会“落地生根”,开辟肿瘤的分基地。

但限于科研条件,当时的科学家们对循环肿瘤细胞的认识并不深。这也难怪。要知道,即便是病情进入晚期的癌症患者,体内每10亿个血细胞中,也只能找到1个循环肿瘤细胞。这就好像要在全世界的70亿人口里找到指定的7个人,谈何容易?

本研究的通讯作者Aceto教授在循环肿瘤细胞的研究上做出了不少突破。在2014年,当他还在哈佛医学院进行博士后训练时,就发现癌症转移的关键,不是单个循环肿瘤细胞,而是一堆循环肿瘤细胞组成的“小聚集体”。这个发现同样也发表在了《细胞》杂志上。

加盟巴塞尔大学后,Aceto教授与团队深入研究了这一问题。为了更好地了解这些“小聚集体”如何增加转移风险,研究人员们做了一个比对,看看它们与单个循环肿瘤细胞有何不同。从乳腺癌患者体内,以及小鼠模型中,研究人员们获取了实验的材料。DNA甲基化水平的分析结果,表明了一个非常有趣的现象——在循环肿瘤细胞小聚集体内,包括OCT4、NANOG、SOX2、以及SIN3A等与增殖有关的转录因子的结合位点,甲基化水平明显降低。

这意味着什么呢?研究人员们在论文中指出,这说明这团癌细胞更具有“胚胎干细胞”的活性,让它们能够不断增殖,并且促进转移。

随后,这群科学家们灵光一闪,想到了一个抗癌的新方法。既然癌症转移的关键是循环肿瘤细胞聚集成小聚集体,如果打散这些小聚集体,是不是就能抑制这些细胞的增殖能力,降低转移风险呢?

为了回答这个问题,研究人员们做了一个大规模的药物筛选,所筛选的对象是2486个美国FDA已经批准上市的化合物。筛选结果表明,能够抑制钠钾泵(Na+ / K+ ATPase)功能的一些化合物,可以有效打散这些循环肿瘤细胞。被打散的单个细胞里,DNA关键位点的甲基化修饰水平果然也得到了恢复。

更令人欣喜的是,这种疗法在小鼠模型中起到了很好的预防效果。在一项实验中,科学家们在小鼠体内注射了癌细胞,诱发肿瘤生长。在肿瘤生长的14周后,科学家们每日给小鼠使用一种叫做ouabain的药物,连续治疗3周。随后,他们开始评估这种药物对于“打散”癌细胞团,以及预防转移的效果。

研究表明,在总体循环肿瘤细胞数目不变的情况下,ouabain显著减少了循环肿瘤细胞自发成团的频率。与之对应,单个循环肿瘤细胞出现的频率有所提高。但这些散兵游勇成不了大气候。研究人员发现,在ouabain的治疗下,小鼠的癌症转移风险降低了80倍!这是一个巨大的成就。

“我们想到了不同的治疗策略。我们开始寻找那些不会杀死癌细胞,而只是让他们分散的药物。” Aceto教授说道。鉴于这项动物实验所取得的惊人成绩,研究人员们期待尽早开展人体临床试验。而他们所针对的第一个疾病,就是容易出现转移的乳腺癌。

乌苯苷:

乌苯苷是一种甾类糖苷,别名g-毒毛旋花苷。用作强心剂。在原产地也用于制作毒箭。从产于非洲的夹竹桃科的羊角拗属(St-rophanthus gratus)的种子或同科的Ac-ocanthera ouabaio的木材中获得。与从毒毛旋花(S.kom-be)得到的k-毒毛旋花苷有所区别。乌本苷的提取物可以降低精子的活力,或将以此为原料制成男用口服避孕药
