腰硬穿刺要点总结，麻醉医生必知！

椎管内穿刺对很多初学者来说，每次都是战战兢兢，能否穿刺到位全靠运气；看上级医生穿刺行云流水，且自觉对每个步骤都了然于胸，但一到自己上手仍是丢三落四，造成这样的结果多数是对腰硬穿刺的细节缺少总结。
以下是我从自身经历出发，总结出每一步穿刺自认为很关键的细节，与大家分享，如有相左或遗漏，欢迎留言讨论。

消毒的原则，是以穿刺点为中心至少15cm消毒范围，消毒过程中不能留白且每次消毒范围递减，消毒刷不宜太湿，否则上侧的消毒液会流经穿刺点；
对于消毒方法，我曾经做过一个200多人的问卷调查，发现选择最多的是以上三种，方法并无好坏，关键是你是否严格遵循消毒原则。

置入腰麻针，需要刺破硬膜和蛛网膜，直接刺入很多时候不能刺穿，特别是年轻患者硬膜柔软；记得自己刚开始独立操作，连续三位患者的联合麻醉都是硬膜外进入顺利、腰麻针不出脑脊液，但上级医生却一针到位；刚开始完全不理解，后来自己仔细推敲，发现其实前三次腰麻针已经顶住了硬膜，但就是没有刺破，第四次旋转刺入就一次成功。
此外，腰麻针是侧孔注药，所以明显有落空感却不出脑脊液，可能是侧孔刚好不在脑脊液内，应尝试不同方向；此外，推注腰麻药是最关键的操作，手背靠住患者背部稳定腰麻针非常重要，同时在推注过程中应回抽至少三次，明确局麻药进入蛛网膜下。
