【1】 贫血常见于心力衰竭患者中,且常与铁缺乏相关

流行病学上,关于慢性心力衰竭门诊病人贫血和铁缺乏的患病率、相互关联和结果方面的详细信息尚缺乏。本研究旨在大量临床上疑似心力衰竭的病人中调查贫血和铁缺乏的情况。结果发现,贫血常见于心力衰竭患者中,且常与铁缺乏相关。贫血与铁缺乏都与全因死亡率和心血管疾病死亡率的增加有关,或是这些患者的治疗靶点。

研究人员收集了同一家社区门诊就诊的疑似心力衰竭病人的临床数据,包括血红素,血清铁,运铁蛋白饱和度和血清铁蛋白浓度。共纳入4456名患者,年龄的中位数为73岁,2696名男性,1791名患者左心室收缩功能不全(LVSD)。

无LVSD的患者中,1172名患者血浆N端脑钠肽浓度超过400pg/mL,841名患者低于400pg/mL,652名未检测。总的来说,1237名患者患有贫血,心力衰竭的病人中无论是否伴有LVSD,其贫血的患病率较高。根据定义,贫血患者中,270-425名患者存在铁缺乏,非贫血患者中,260-624名患者存在铁缺乏。多变量分析中,低血红素和血清铁浓度与较高的全因死亡率和心血管疾病死亡率独立相关。(文章详见——JAMA Cardiol:贫血常见于心力衰竭患者中,且常与铁缺乏相关



【2】 Nutr J:补充维生素D会影响铁含量吗?

不管是维生素D还是铁缺乏都在全球范围内普遍存在,并有研究指出这些缺乏之间存在一定关系。该随机对照试验结果表明,对低维生素D状态参与者持续16周每日补充10或25μG维生素D3不影响血红蛋白或其他铁状态标记物的水平。

研究共纳入251名参与者,随机分配接受每天口服10μg、25 μg维生素D3或安慰剂,持续时间16周。在基线和16周后检测参与者血液中25羟维生素D(s-25(OH) D)、血清铁蛋白、血红蛋白、血清铁水平。

最终214名(占随机对照组的86%)符合条件的参与者完成了干预。结果显示,接受维生素D3和安慰剂组的参与者的血清铁蛋白、血红蛋白、血清铁、转铁蛋白饱和度没有明显差异。干预后参与者血清25羟维生素D从基线的29 nmol/L增加到49 nmol/L,安慰剂对照组则没有什么变化。(文章详见——Nutr J:补充维生素D会影响铁含量吗?

【3】维生素D可影响铁循环

体外研究表明维生素D可以降低铁调素的表达,降低单核细胞释放促炎性细胞因子。然而,维生素D对健康人铁回收影响相关数据仍缺乏。我们的目的是研究大剂量维生素D3对健康成人血浆铁调素、炎性细胞因子和血清铁蛋白浓度的影响。结果表明,维生素D在通过独立促炎症标志物变化上调节铁循环中占有一席之地。

此试点双盲安慰剂对照试验纳入28名健康成年人,随机分为一次性口服250000 IU的维生素D3或安慰剂。基线时期,血浆25羟维生素D[25(OH)D]、铁调素、促炎性细胞因子和铁蛋白浓度在两组之间没有差异,并且两组均超过70%的参与者缺乏维生素D(25(OH)D < 20 ng/mL)。1周后,维生素D3组参与者的血浆铁调素浓度下降了73%;安慰剂组没有显著变化。两组的血浆细胞因子和铁蛋白浓度均无显著变化。(文章详见——Clin Nutr:维生素D与铁循环



【4】血清铁浓度与癌症风险、死亡率相关,且存在性别差异

虽然铁的致癌潜力已被证明,但观察性研究的证据所表明的,血清铁变量和癌症的关联结果是不一致的。此项研究探究是否较高的铁浓度增加患癌症的风险。

研究纳入了1597名男性和1795名女性,年龄25-79岁,基线无癌症病史,血清铁蛋白浓度≥20 μg/L。校正混杂因素后发现,对于女性,较高的血清铁浓度和转铁蛋白饱和度与非皮肤癌风险的增加有关:铁 HR=1.83;转铁蛋白饱和度 HR=1.68;这些癌症中包括女性乳腺癌;转铁蛋白饱和度 HR=1.90。

此外转铁蛋白饱和度与更大的癌症死亡风险有关。对于男性,更高的铁浓度与非皮肤癌风险降低有关,包括直肠癌。没有发现妇女血清铁与结直肠癌风险之间的关联。此外血清铁蛋白与癌症风险或癌症死亡无关。

【5】1/3的孕妇缺铁,孕妇要重视铁的补充

缺铁性贫血是一种常见的贫血类型,WHO指出,全球超过30%的民众存在贫血,主要表现为缺铁性贫血。圣皮埃尔大学医院内分泌科Kris Poppe博士指出,孕妇出现缺铁性贫血对母亲和胎儿均不利。铁有助于甲状腺过氧化物酶(TPO)的合成,这对甲状腺激素的合成至关重要。孕妇需要产生足够的甲状腺激素以利于胎儿大脑的发育,特别是在孕早期尤为重要。


研究中,Poppe博士和同事着手探究出现缺铁性贫血的孕妇的人数及与甲状腺疾病发展之间的关系。研究人员对1900名孕早期孕妇进行了随访调查。结果发现,35%的孕妇出现了铁缺乏,其中10%的孕妇存在甲状腺自身免疫性疾病,在无铁缺乏的孕妇中,6%的孕妇患有甲状腺自身免疫性疾病

【6】乳铁蛋白能降低早产儿感染风险

根据最近Journal of Pediatrics上的研究结果,Talactoferrin(重组人乳铁蛋白)能降低早产儿医院获得性感染的风险。

密苏里堪萨斯大学儿童健康系的Michael P. Sherman和同事进行了一项随机、双盲、安慰剂对照试验,涉及120例体重750克至1500克的早产儿,研究对象被分为两组:在28天的生活中,每12个小时使用150 mg talactoferrin (Agennix),或安慰剂对照组。

研究结果表明:talactoferrin组比对照组感染发生率低一半。此外,与对照组相比,talactoferrin组早产儿的肺炎、尿路感染和血行感染的发生率更低。进一步分析表明,体重不到1公斤的早产儿中,talactoferrin组和对照组革兰氏阴性菌感染的案例分别为0和3例。(文章详见——J Pediatr:乳铁蛋白能降低早产儿感染风险



【7】Diabetes Care:铁代谢的生化标志物与2型糖尿病的关系

有观察性研究发现铁蛋白和2型糖尿病有关,机体较高的铁储备可能参与了2型糖尿病的发展。然而铁蛋白不会被铁储备所影响,因此相对于其他标志物,铁蛋白并不能完全反映机体的铁代谢情况。研究人员通过观察铁蛋白、转铁蛋白饱和度(TSAT)、血清铁和转铁蛋白与2型糖尿病的发生,研究铁在2型糖尿病发病机制中的作用。

EPIC- InterAct研究中对已明确诊断为2型糖尿病并长期随访的12403例患者、以及有代表性的16154例亚群进行研究。研究人员观察了11052例2型糖尿病患者以及15182例亚群中2型糖尿病的发生与铁蛋白、转铁蛋白饱和度(TSAT)、血清铁和转铁蛋白的联系。

研究结果显示,校正混杂因素后,较高水平的铁蛋白和转铁蛋白与2型糖尿病的风险增加有关。在女性中,较高的TSAT (≥45% vs <45%) 与2型糖尿病风险降低有关,但在男性中无统计学差异。血清铁与2型糖尿病无明显相关性。在较瘦的人群中,铁蛋白与2型糖尿病有更强的相关性。(文章详见——Diabetes Care:铁代谢的生化标志物与2型糖尿病的关系



【8】IBD患者绿茶和膳食铁不宜同时摄入

绿茶因其强大的抗氧化功能而具有很多健康益处,但是一项针对炎症性肠病模式小鼠的新研究提示着同时摄入绿茶与膳食铁可能实际上降低绿茶的益处。

在这项研究中,Vijay-Kumar和同事们发现,表没食子儿茶素没食子酸酯(EGCG)---绿茶中的一种主要化合物---强效地抑制炎症发生期间白细胞释放的一种被称作髓过氧化物酶的促炎性酶。利用EGCG让髓过氧化物酶失活可能也有助于缓解炎症性肠病(IBD)中的炎症。但是当同时摄入EGCG和铁时,与铁结合后,EGCG丧失了它的抑制髓过氧化物酶的能力。

让事情更加复杂的是,研究人员还发现一种被称作脂质运载蛋白的宿主蛋白也能够让EGCG失活,其中在炎症性疾病中,该蛋白大量存在。

IBD的特征是消化道慢性炎症,这会导致出血性腹泻、疼痛、疲乏、体重降低和包括铁缺乏/贫血在内的其他症状。医生经常给IBD病人开铁补充剂药方。在这种情形下,同时摄入绿茶和铁补充剂可能帮倒忙,这是因为这两种营养物结合在一起而相互抵消。(文章详见——Am J Pathol:IBD患者绿茶和膳食铁不宜同时摄入

【9】口服VS. 静脉注射铁补充治疗可明显改变IBD患者的肠道菌群及代谢

铁缺乏症是IBD患者的一个常见并发症,口服补充铁可恶化IBD患者的症状。我们进行了此项非盲临床试验以对比口服(PO)和静脉注射(IV)铁补充疗法(IRT)的效果。结果发现,IBD患者其肠道菌群的种类及组成的改变与铁治疗有关。尽管口服与静脉注射铁补充治疗的临床结果相似,但是两种治疗方法对微生物种系以及粪便代谢物的影响不同。

研究共纳入了31名克罗恩病(CD)患者,22名UC患者,以及19名铁缺乏对照患者。随机分组,所有参与者接受硫酸铁(PO)或蔗糖铁(IV)治疗3个月。干预治疗前后评估所有参与者的临床参数,粪便中微生物群落以及代谢状况。

PO和IV治疗都可改善铁缺乏的症状,但是IV组患者存在较高的铁蛋白水平。铁补充治疗方式并不影响疾病活动性的变化。IBD患者的粪便样本存在明显地个体差异,微生物的种系丰富度及Clostridiales的比例减小。代谢产物分析的结果同样显示UC和CD与对照贫血患者不同。约一半的参与者在经IRT后微生物的种类发生了明显的改变,但是CD患者似乎最易发生改变。尽管由于IRT治疗,个体微生物种系发生了特异性的改变,但是PO治疗可降低Faecalibacterium prausnitzii, Ruminococcus bromii, Dorea sp. 以及Collinsella aerofaciens的丰富度。在代谢物水平,清晰的IV特异性和PO特异性指纹非常明显,其变化影响了胆固醇来源的宿主基质。(文章详见——Gut:口服VS. 静脉注射铁补充治疗可明显改变IBD患者的肠道菌群及代谢



【10】葡萄糖酸钠铁和蔗糖铁在血液透析患者中的安全性比较

葡萄糖酸钠铁和蔗糖铁拥有不同的药理学特性,研究者对血液透析病人进行了一项回顾性队列研究,比较两种药物的安全性。结果表明,虽然葡萄糖酸钠铁和蔗糖铁具有不同的药理学特征,但短期的安全性差异不大。不过对使用血液透析导管的患者而言,葡萄糖酸钠铁可轻度降低感染相关事件风险。

该研究共纳入了66207名血液透析患者,11.4%的患者使用葡萄糖酸钠铁,48.9%的患者使用蔗糖铁,还有39.7%的患者没有使用相关铁配方。研究者以随访3个月内的全因死亡率、感染相关住院和死亡率、心血管相关住院和死亡率为结局指标。

研究结果显示,大部分结局指标风险在葡萄糖酸钠铁组和蔗糖铁组没有差异,不过使用血液透析导管的患者中使用葡萄糖酸钠铁组其感染相关死亡和感染相关住院情况更少。此外,静脉给药会增加葡萄糖酸钠铁组和蔗糖铁组的感染相关事件风险。(文章详见——Am J Kidney Dis:葡萄糖酸钠铁和蔗糖铁在血液透析患者中的安全性比较
