早产是指在妊娠37周前分娩,是新生儿发病和死亡的主要原因,全世界每年有近1500万例早产。大多数早产是由自发性早产造成的。对早产妇女进行及时干预可以大大改善早产儿的健康状况。

这些措施包括在有适当新生儿护理设施的中心分娩,在出生后7天内使用产前皮质激素促进肺部成熟,以及围产期使用硫酸镁保护神经。然而,确立早产的诊断是非常具有挑战性的,因为早产的症状和体征很常见,而且可能是非特异性的。

同时,大量有早产症状的妇女被过度诊疗——不必要的预防性治疗会对产妇和新生儿造成不良影响。三分之一的产前入院病人被怀疑为早产,给医疗服务带来了巨大的经济负担,并给妇女和家庭带来负面的经济和情感影响。

对出现早产的妇女进行及时的干预,可以大大改善早产婴儿的健康结果。然而,确定这样的诊断是非常具有挑战性的,因为早产的症状和体征很常见,而且可能是非特异性的。为此,英国爱丁堡大学Usher研究所的专家旨在开发和外部验证一个风险预测模型:结合临床风险因素,使用阴道液胎儿纤连蛋白的浓度(定量fFN)来预测自发性早产,并评估其成本效益。相关结果发表在最新的PLOS Medicine杂志上。

纳入分析的孕妇为妊娠22+0至34+6周,且有早产的迹象和症状。主要结果是定量fFN检测后7天内的自发性早产。该风险预测模型是在对5项欧洲前瞻性队列研究(2009-2016年;1783名妇女;平均年龄29.7岁;BMI中位数24.8 kg/m2;67.6%为白人;11.7%为吸烟者; 10.4%为多胎妊娠;139[7.8%]为7天内自发性早产)的个体参与者数据(IPD)荟萃分析中开发并进行内部验证。

随后,该模型在英国26个中心的前瞻性队列研究中得到了外部验证(2016-2018年;2924名妇女;平均年龄28.2岁;BMI中位数25.4 kg/m2;88.2%为白人;21%为吸烟者; 3.5%为多胎妊娠;85 [2.9%]为7天内自发性早产)。开发的7天内自发性早产的风险预测模型包括定量的fFN、目前吸烟、非白种人、多胎妊娠。

开发的用于预测7天内自发性早产的QUIDS风险预测模型(A),并通过IPD荟萃分析进行内部验证(B)。

经过内部验证,调整后的曲线下面积为0.89, R2为35%。在英国前瞻性队列人群中进行外部验证,曲线下面积为0.89,R2为36%(95% CI:34%至38%)。校准曲线表明,在预测0%至10%的范围内,预测和观察到的风险接近一致,但在更高的风险下有一些误判(预测不足)。

进一步分析显示,尽管有误判,但在风险高达约15%的阈值下,该模型的净效益高于 "全部治疗 "或 "不治疗 "的策略。经济分析发现,与使用定性的fFN相比,在7天内早产风险≥2%的入院和治疗阈值下,预后模型具有成本效益。

在前瞻性队列数据中,QUIDS模型验证的预测与观察风险的校准图。

综上,包括阴道fFN浓度和临床风险因素的风险预测模型在预测7天内的自发性早产方面显示出良好的性能,并有可能为有早产威胁的妇女的管理决策提供参考。
