Heart:心源性猝死患者心电图与心肌纤维化的关系

心肌纤维化与QRS波延长、深Q波、T波倒置和QRS波碎裂有关。这些结果为识别纤维性心肌病和SCD风险患者提供了潜在有用的无创早期指标。

降低心源性猝死(SCD)发生率的一个主要挑战是识别有风险的患者。心肌纤维化与SCD风险之间存在关联,但在一般人群中很难确定。近日,心脏病领域权威杂志Heart上发表了一篇研究文章,研究人员旨在寻找SCD患者心肌纤维化的心电图标记物。

研究人群来自Fingesture研究,该研究收集了1998年至2017年期间芬兰北部5869例连续尸检的SCD患者数据。在尸检时从心脏组织取样,测定纤维化程度,并将其分为四组:(1)无纤维化、(2)弥散性轻度纤维化、(3)中度斑片状纤维化和(4)大量纤维化。研究人员收集了1100名SCD受害者的心电图。

研究对象的平均年龄为66±13岁,其中75%为男性。心电图QRS时程与纤维化程度相关(p<0.001,β=0.153)。碎片化QRS波、病理Q波和T波倒置的发生率与纤维化程度的增加相关(p<0.001)。非缺血性和缺血性心脏病均可观察到去极化异常。只有缺血性SCD患者中复极化异常才有统计学意义。(1)组中75.3%、(2)组中73.7%、(3)组中88.5%和(4)组91.7%的患者出现了异常心电图(p<0.001)。

由此可见,心肌纤维化与QRS波延长、深Q波、T波倒置和QRS波碎裂有关。这些结果为识别纤维性心肌病和SCD风险患者提供了潜在有用的无创早期指标。
