在2型糖尿病中，体重指数（BMI）可能是比冠心病更强的心力衰竭风险因素。但是先前研究尚未通过BMI和血糖水平明确与年龄和性别相匹配的一般人群比较者相比2型糖尿病患者急性心肌梗塞和心力衰竭的相对和绝对风险的依据。

 

我们在1998年至2012年期间从瑞典国家糖尿病登记处确定了181045名患者，并按年龄，性别和乡村配对了1538434名无糖尿病的普通人群对照者，所有患者均未患上重大心血管疾病。通过与瑞典住院登记处的联系，对病例和对照者的结果进行了追踪。

 

结果显示，在5.7年的平均随访时间中，患者和对照者中有28 855例急性心肌梗塞和33 060例心力衰竭病例。与低BMI的肥胖患者相比，肥胖者的过度风险（高于没有血红蛋白A1c和BMI数据的对照者的风险），心力衰竭的发生率和危险比明显身高，血糖控制不佳的肥胖者（BMI≥40kg/m2）心力衰竭的风险是对照者的7倍（参考水平）。相比之下，对于急性心肌梗塞，在血糖控制不佳的患者绝对风险和相对风险最高，而BMI升高则无额外风险。

 

总之，该研究结果表明，BMI是2型糖尿病患者心力衰竭而不是急性心肌梗塞的强独立危险因素。
