Thyroid:甲状腺功能改变预示着老年人认知能力下降

与生理预期相反,在85岁的老人中,血清TSH下降与游离甲状腺激素随着时间的推移而降低。随着时间的推移血清TSH的降低预示着晚年认知能力的下降。TSH浓度下降是晚年认知障碍的生物标志物。

在老年人的观察性研究中,低血清TSH水平与认知障碍风险增加有关,但其背后的机制尚未明确。

近日,甲状腺疾病领域权威杂志Thyroid上发表了一篇研究文章,研究人员利用纽卡斯尔85岁以上老年人研究的前瞻性数据,探究了老年人群中甲状腺状态改变与认知障碍之间的关系。研究人员对85岁人群进行健康状况和甲状腺功能评估。

研究人员对642名游离甲状腺激素正常且TSH水平在0.1-10mU/L之间的参与者进行了全面多维健康检查和重复甲状腺功能测量,获得了完整的数据。研究人员采用迷你精神状态检查(MMSE)和认知药物研究系列检查评估了认知能力,并采用线性混合、logistic回归和Cox比例风险模型,评估了认知能力与基础血清TSH、游离T4和游离T3水平和3年变化值之间的相关性。

甲状腺功能与认知能力的关系

3年期间,血清TSH下降与游离T4和游离T3降低相关,并与5年期间发生认知障碍风险增加相关[OR为1.77 (95%CI: 1.19-2.61);p=0.004)。MMSE评分降低越多,TSH下降程度越大(p<0.001)。TSH升高的受试者体内游离T4浓度稳定。

由此可见,与生理预期相反,在85岁的老人中,血清TSH下降与游离甲状腺激素随着时间的推移而降低。随着时间的推移血清TSH的降低预示着晚年认知能力的下降。TSH浓度下降是晚年认知障碍的生物标志物。
