Br J Anaesth:临床相关剂量电离辐射与氯胺酮共同暴露可能对婴幼儿成年后认知功能产生影响

医疗保健中常使用放射学方法筛查、诊断及治疗疾病,且对婴幼儿进行放射学检查时,通常使用麻醉或镇静来减轻疼痛与缓解压力。有研究表明电离辐射(IR)与氯胺酮对发育期神经系统可产生神经毒性,因此本研究主要探讨电离辐射(IR)与氯胺酮共同暴露对新生小鼠成年后认知功能的影响。

背景与目的:医疗保健中常使用放射学方法筛查诊断及治疗疾病,且对婴幼儿进行放射学检查时,通常使用麻醉或镇静来减轻疼痛与缓解压力。有研究表明电离辐射(IR)与氯胺酮对发育期神经系统可产生神经毒性,因此本研究主要探讨电离辐射(IR)与氯胺酮共同暴露对新生小鼠成年后认知功能的影响。

方法:随机将出生后10天的新生小鼠分为三组:对照组、氯胺酮组、氯胺酮联合IR组;使用氯胺酮7.5mg/kg进行麻醉,1h后用50-200 mGyγ射线(137Cs)全身照射小鼠,在2、4、5月龄时测试小鼠行为学指标,6月龄时分析小鼠大脑皮层与海马蛋白表达量。

结果:与对照组和单一药物暴露组比较,氯胺酮与IR联合暴露的小鼠在行为学测试中缺乏习惯性;水迷宫试验中,联合暴露组小鼠学习及空间记忆能力减退;联合暴露组小鼠大脑皮质区tau蛋白的水平升高;与对照组比较,单一药物暴露组小鼠的行为学试验指标及蛋白表达水平均无变化。

结论:新生小鼠单一剂量麻醉药物暴露不会影响成年后认知功能,但相同剂量的麻醉药物(氯胺酮)与IR共同暴露会加重发育期神经毒性;本研究表明婴幼儿期低剂量电离辐射单独暴露可降低危险人群认知功能改变的可能性。
