European Radiology:深度学习在检测和分期慢性COPD方面的价值

随着计算机断层扫描(CT)在肺癌筛查中的应用越来越多,因此利用这些扫描来进行COPD筛查成为可能。

慢性阻塞性肺病(COPD)是一个世界性的公共卫生挑战,发病率以及相关残疾、甚至死亡的风险一直居高不下。对COPD的准确诊断对于及时的治疗干预以改善患者的生活质量和减少未来病情加重的风险至关重要。研究报告称,估计超过40%的COPD患者仍未确诊,特别是在发展中国家尤为明显。此外,先前的研究报告称,仅使用肺活量测定法进行诊断时,COPD可能被误诊甚至漏诊。因此,临床上迫切需要建立一个更精确的诊断手段来提供的COPD检测和评估。

随着计算机断层扫描(CT)在肺癌筛查中的应用越来越多,因此利用这些扫描来进行COPD筛查成为可能。越来越多的证据表明,CT的定性和定量成像分析在COPD的诊断和分层中具有潜力。因此,基于CT的成像可以改善COPD的检测和评估。然而,传统的人工检查CT图像往往耗时且主观,这限制了其在大规模COPD筛查中的应用。现阶段基于深度学习(DL)的人工智能(AI)的最新进展使医学图像的直接解读成为可能。

近日,发表在European Radiology杂志的一项研究开发了一个基于多实例学习(MIL)模型进行COPD检测,为临床大规模快速、准确的诊断COPD患者提供技术支持。

本研究建立了一个大型的、高度异质性的数据集,包括从中国四家大型公立医院的门诊、住院和体检中心回顾性招募的1393名参与者。所有参与者都接受了吸气式胸部CT扫描和肺功能测试。收集了每个参与者的CT图像、肺活量数据、人口统计信息和临床信息。利用837名参与者的CT扫描图像,训练了一个基于注意力的多实例学习(MIL)的COPD检测模型。利用从国家肺部筛查试验(NLST)队列中获得的620张低剂量CT(LDCT)扫描对COPD检测进行了外部验证。进一步开发了一个多通道的三维残余网络,在确认的COPD患者中对GOLD分期进行分类。 

用于COPD检测的基于注意力的MIL模型在内部测试集上取得了0.934(95% CI: 0.903, 0.961)的受试者工作特征曲线下面积,在从NLST获得的LDCT子集上取得了0.866(95% CI: 0.805, 0.928)。多通道三维残余网络能够使用GOLD量表对测试集中76.4%的COPD患者(423/553)进行正确分级。

本研究开发并验证了一种用于检测和分期COPD的CT-DL方法,该模型方法达到了预期的诊断性能,并可作为COPD病例诊断的有力工具,为临床医生提供有用的指标和临床相关发现,进一步改善患者的管理和后续治疗。
