研究人员对184例福尔马林固定石蜡包埋(FFPE)肿瘤组织和基线对照样本中的76个微卫星位点(MSI-NGS)进行了扩增性靶向测序。使用生物信息学工具MSIcall计算基于对齐序列读取的定量值并评估MSI状态。此外,我们还检验了MSI-NGS和MSI-PCR/IHC结果的一致性。通过受试者工作特征(ROC)曲线分析来评估诊断的准确性、敏感性和特异性。对于验证队列,我们研究了额外的50个肿瘤样本以确定MSI状态。

在184个肿瘤样本中,MSI-PCR和IHC分析将161个肿瘤分类为MSS/pMMR,23个分类为MSI-H/dMMR。利用MSI-NGS与MSIcall相结合,我们对25种癌症的MSI状态进行了高准确度(98.9%)、特异性(91.3%)和敏感性(100%)的预测。该方法的ROC曲线下面积(AUC)值为0.9986。此外,我们还利用额外的50个样本进行了验证,取得了100%的一致性结果。

本研究证明了新开发的MSI-NGS与MSIcall准确地确定了FFPE肿瘤组织的MSI状态,单独对肿瘤标本进行了彻底的测序,没有患者匹配到正常对照。这种方法可以应用于所有类型的实体肿瘤,以确定免疫肿瘤治疗的应答者。
