近日，中国接连发布重度雾霾预警，使近两年我们好不容易改善的空气质量再度跌入谷底。
世卫组织的数据显示，在全世界，十分之九的人呼吸不健康的空气。如今，空气污染是造成早期死亡的最大环境风险，每年因诱发心脏病、卒中、肺癌、糖尿病和呼吸系统疾病导致700万人过早死亡。这远远超过了艾滋病、结核病和疟疾造成的死亡总数。而儿童是受空气污染伤害最严重的群体。
近期便有研究揭示，儿童早期暴露于受污染空气环境还会改变基因，从而导致他们在成年后更易患心脏病或其他疾病。
城市地区的大部分可吸入颗粒物（PM）是由汽车尾气排放、非道路设备和工业设施燃烧煤炭或石油等化石燃料直接形成的。其他来源包括灰尘、气体和蒸汽的二次颗粒形成等。
美国斯坦福大学主导的一项新研究显示，儿童暴露于空气污染，如野火烟雾和汽车尾气，只要一天，就可能注定成年后心脏病和其他疾病的发病率更高。
这项发表在《自然科学报告》（scientific reports）上的分析，首次在单细胞水平上研究了空气污染的影响，并同时关注儿童的心血管和免疫系统。它证实了之前的研究，即糟糕的空气会以一种可能影响长期健康的方式改变基因调控——这一发现可能会改变医学专家和家长对儿童呼吸污染空气的看法，并为那些暴露于长期空气污染的人的临床干预提供参考。

研究人员研究了美国加利福尼亚州弗雷斯诺市的一个以西班牙裔为主的6-8岁儿童群体（n=221），该市由于工业、农业和野火等原因，空气污染水平居全国之首。利用在弗雷斯诺中央空气监测站测量的连续日污染物浓度、定期空间采样的日浓度以及气象和地球物理数据的组合，研究小组估计了每次参与者访问前1天、1周以及1、3、6和12个月的平均空气污染暴露情况。当与健康和人口统计调查问卷、血压读数和血液样本相结合时，数据开始描绘出一幅令人不安的画面。
研究发现空气污染暴露与免疫调节基因的甲基化、相关免疫细胞类型的蛋白质表达改变和血压升高之间存在关联。
短期和长期暴露于PM2.5（1至3个月）、CO（1天至12个月）和O3（1天至6个月）是预测大多数免疫调节基因甲基化的关键变量。在进行多变量调整的回归分析时，免疫调节基因的CpG甲基化与模型分析中突出显示的大多数空气污染暴露独立相关。

在多变量模型中，最近暴露于PM2.5、CO、O3和PAH456是免疫细胞谱的主要预测指标。另外，在进行多变量调整线性回归时，显示较高的收缩压与3个月的NOx、1天的NO2和6个月的PAH456暴露独立相关（P≤0.043；P≤0.043；P≤0.07）。

这些研究结果表明，生命早期接触污染可能与多种后果相关，可能会影响成年后的长期健康。一氧化碳和臭氧随着时间的推移与甲基化的增加有关，这是DNA分子的一种改变，可以在不改变其序列的情况下改变其活性。这种基因表达的改变可能会遗传给后代。
研究的主要作者、斯坦福大学过敏和哮喘研究中心的空气污染和健康研究主任Mary Prunicki说，“看起来，即使是短暂的空气污染暴露，实际上也可以改变儿童基因的调节和表达，也许会改变血压，导致以后的疾病风险增加”。
另外，研究还发现空气污染暴露与单核细胞的增加有关，单核细胞是在动脉斑块堆积中起关键作用的白血球，可能会使儿童在成年后易患心脏病。麻省总医院（MGH）的研究人员于同一时期发表在《欧洲心脏杂志》（European Heart Journal）上的研究也证实，空气污染中的细颗粒物质可通过激活骨髓中炎症细胞的产生，对心血管健康产生有害影响，最终导致动脉炎症。这项工作得到了美国国立卫生研究院（NIH）的支持。
这项回顾性研究纳入了503名无心血管疾病或癌症的患者，他们出于各种医学原因在MGH进行了影像学检查。科学家使用从距离每个参与者居住地最近的美国环境保护局的空气质量监测器中获得的数据来估算参与者暴露的年度平均细颗粒物水平。
结果显示，在4.1年的中位随访期间，有40个人经历了重大的心血管事件，例如心脏病发作和卒中，在其家庭住址中具有较高水平的细颗粒物的参与者中，发生风险最高。即使考虑了心血管危险因素、社会经济因素和其他主要混杂因素，他们的风险也有所增加。评估内部器官和组织状态的影像学检查表明，这些参与者的骨髓活性也更高，表明炎症细胞的产生（白细胞的生成）增加，并且动脉炎症增加。

研究显示，空气污染暴露引发的高骨髓活性和动脉炎症诱发心血管疾病事件的占比达29％。这些发现暗示着空气污染暴露是心血管疾病尚未被认识的危险因素。
空气污染对儿童的严重伤害不只在于心血管疾病，还可能导致心理健康问题。来自美国哥伦比亚大学的研究人员称，家庭早期生活压力和产前空气污染暴露升高的儿童，表现出注意力和认知障碍的症状加重。产前暴露于空气污染中常见的神经毒物，似乎会放大早期生活压力对儿童心理健康的影响。而这些影响与空气污染暴露的剂量敏感标志PAH有关。

早期的研究使用相同的纵向队列数据，发现产前暴露于空气污染，可显著增加儿童的多动症症状。另一项研究发现，空气污染和贫困共同降低了儿童的智商。
另外一种可怕的伤害是，室外空气污染物会增加患癌症的风险。2012年6月，世卫组织下的国际癌症研究机构（IARC）将柴油机废气从“可能对人类致癌”（第2A组）重新分类为“对人类致癌”（第1组）。
这些证据都表明，空气污染的伤害是长期的，甚至会改变人类表观遗传，这对社会发展的影响可想而知。减少空气污染风险因素的暴露，对改善身心健康，甚至对人类社会的可持续发展都是有益的。
环境污染加重往往伴随着工业化速度发展过快。未来寻求清洁能源、加快绿色出行覆盖率仍是改善空气质量的主要举措。而在目前可以采取的措施中，戴普通口罩（例如外科手术口罩和N95口罩）可以过滤空气中的较大颗粒，但有些研究表明，外科手术口罩对过滤小于10微米的PM无效，更不用说过滤PM2.5了。在防止吸入气态空气污染物（例如一氧化碳、二氧化氮、臭氧和二氧化硫）方面无用。N95口罩过滤PM的效率取决于其滤芯功效。而且，长时间佩戴会导致呼吸困难。
除采取外部措施，服用药物是否能有针对性地改善空气污染带来的身体伤害？可喜的是，确实有研究发现，维生素B可减少空气污染引起的心血管疾病的影响。来自哥伦比亚大学的研究显示，暴露于PM2.5两小时对心率、和白血球计数有实质性的生理影响。而这些影响在补充四周B族维生素后几乎可以逆转。服用维生素B补充剂的健康非吸烟者几乎逆转了对其心血管和免疫系统的任何负面影响，将空气污染对心率的影响削弱了150%、白细胞总计数削弱了139%、淋巴细胞计数削弱了106%。

但这项研究只是来自轻度污染城市的健康成年人，因此研究结果可能无法普遍适用于对污染引起的心血管影响风险较高的人群，包括儿童、老年人、已有心血管疾病的个体以及居住在严重污染地区的个体。
因此，目前若想真的防止空气污染伤害公众健康，污染监管仍然是公共卫生保护的支柱。总不能在发生空气污染事件时，让所有人都出走。
