Clin Chem:高灵敏度心肌肌钙蛋白I可检测早期诊断急性心肌梗死

本研究的目的是验证Beckman Access高敏感性心肌肌钙蛋白I (hs-cTnI)检测的临床表现。研究人员收集急性心肌梗死(AMI)症状的急诊患者。由2名独立的心脏病专家进行集中判定诊断结果,所有临床信息包括心脏成像两次:第一次使用系列hs-cTnT (Elecsys, primary analysis),第二次使用hs-cTnI (Architect, secondary analys

本研究的目的是验证Beckman Access高敏感性心肌肌钙蛋白I (hs-cTnI)检测的临床表现。研究人员收集急性心肌梗死(AMI)症状的急诊患者。由2名独立的心脏病专家进行集中判定诊断结果,所有临床信息包括心脏成像两次:第一次使用系列hs-cTnT (Elecsys, primary analysis),第二次使用hs-cTnI (Architect, secondary analysis)以及临床使用的hs-cTn。于患者首次就诊时和1小时后采用hs-cTnI测量。主要目的是直接比较hs-cTnIhs-cTnT Elecsyshs-cTnI Architect检测的ROC曲线下面积(AUC)量化的诊断准确性。次要目标包括hs-cTnI访问特定的0/1-h算法的推导和验证。

研究发现,在1579(15.4%)患者中,有243例经确诊为急性心肌梗死。与hs-cTnT Elecsys比较[0.94 (95% CI, 0.93-0.95;P = 0.12)hs-cTnI AccessAUC0.95 (95% CI, 0.94-0.96),高于hs-cTnI Architect [0.92 (95% CI, 0.91-0.94]。将推导得到的hs-cTnI Access 0/1-h算法(推导队列n = 686)应用于验证队列(n = 680),可排除60%的患者[敏感性为98.9% (95% CI, 94.3-99.8)],排除15%的患者[特异性为95.9% (95% CI, 94.0-97.2)]0/1-h算法排除的患者30天生存率为100%。通过对hs-cTnI的系列测量,在二级分析中得到了证实。

因此,贝克曼hs-cTnI检测的诊断准确性和临床应用价值很高,至少可与罗氏hs-cTnT和阿伯特hs-cTnI检测相媲美。

急性心肌梗死合并难治性心力衰竭,我们是这样成功救治的

2018年8月16日,我院急诊抢救室接诊一名77岁男性患者,该患者以“间断胸闷,胸痛1年,加重5小时”为主诉由当地医院转至我院。患者入院前1年,间断在劳累时出现胸痛,呈闷痛,持续数分钟,未予重视,未诊治;入院前5小时,患者胸痛发作持续不缓解,当地医院完善心电图等相关检查,考虑诊断“急性冠脉综合征(ACS)”,对症治疗症状无缓解,建议转入我院,入院时患者仍有胸痛伴呼吸困难。
