Neurology:大脑微出血发生率与淀粉样蛋白负荷之间的关系

近日,神经病学领域权威取杂志Neurology上发表了一篇研究文章,研究人员旨在明确脑微出血(CMBs)的发生率以及淀粉样蛋白PET负担与事件性CMBs之间的关联。

该研究从2011年10月至2017年8月共招募了651名年龄≥50岁的参与者(男性占55%),他们接受了3T MRI扫描,并分别进行了≥2的T2*-加权梯度回波序列。87%的参与者进行了11C匹兹堡化合物B(PiB)PET扫描。研究人员通过使用分段指数模型来计算特定年龄的CMB发生率。在考虑年龄、性别、血管危险因素和APOE的影响后,研究人员使用结构方程模型(SEM)评估了淀粉样蛋白负荷量和基线CMB情况对未来CMB的影响。

参与者在进行基线MRI检查时的平均年龄(SD)为69.8(10)岁,其中111名(17%)参与者的基线CMB≥1。两次扫描之间的时间间隔的平均值(SD)为2.7(1.0)年。CMB的总人群发生率为3.6人/100人每年,并随着年龄的增长而增加:从50岁时的1.5人/100人每年到90岁时的11.6人/100人每年。研究人员使用分段指数模型回归,发病率随年龄的增大和基线CMB增多而增加。SEMs显示:(1)MRI扫描时的年龄增加或携带APOE4等位基因与基线时更多的淀粉样蛋白、更高的淀粉样蛋白相关,尤其是枕叶淀粉样蛋白负荷,反过来又增加了新发脑叶CMB的风险;(2)基线时CMB的存在增加了脑叶CMB的风险,并且具有比淀粉样蛋白负荷更大的效应。

由此可见,年龄和APOE4携带者状态通过淀粉样蛋白负荷起作用,增加了随后发生的脑叶CMB风险,基线CMB是未来CMB的最重要危险因素。
