Oral Oncol:外用氟化物可有效预防成人放射性龋齿

放疗治疗癌症的一个重要手段,近50%的癌症患者都能从放疗中获益,缓解病情或全面改善病情。外用氟化物可用于预防龋齿。然而,头颈区域的放疗存在一些不良反应,如龋齿,称为放射性龋齿。除了积极的口腔卫生维护方案外,通常还建议使用氟化物和洗必泰来预防辐射性龋齿。然而,其有效性,更具体地说,其配方和在接受头颈部放疗的患者中的应用频率仍有争议。本系统综述旨在汇总各种外用氟化物在预防头颈部癌症成年患者的放射性龋齿或细菌生长变化方面的有效性证据。

研究人员检索了三个数据库(PubMed和Google Scholar以及Cochrane),纳入2021年2月前发表的随机对照或非对照试验或准随机试验。两位独立评审员筛选了346份摘要,最终有14篇文章被纳入本次系统评审。评价的主要结果是预防辐射性龋齿或唾液中细菌生长的变化。对根据氟化物配方和与之配套使用的辅助物形成的子组进行了荟萃分析。

 

结果,纳入的研究是高度异质性的。大多数研究发现,不同的氟化物配方对控制放射性龋齿有不同程度的效果,最高可达70%,这取决于干预措施、病人依从性、损耗率和随访期。氟化钠是最常用的龋齿预防剂。荟萃分析显示,氟化钠制剂和其他氟化物制剂之间没有明显差异。与再矿化剂一起使用时,氟化物治疗对预防龋齿没有明显的效果。

综上所述,该研究结果表明,氟化物可以预防放射性龋齿。然而,为了确定其确切的配方、剂量或频率,需要进行更多良好的随机对照试验来加以探究。
