Lancet Infect Dis:抗生素用量与地区抗生素耐药风险

研究认为,抗生素使用剂量增加是抗生素耐药的重要因素,并且会导致一定地理范围内的耐药风险增加

近日研究人员就抗生素耐药的邻里效应进行了考察,以评估初级医疗保健中增加抗生素用量对抗生素耐药性风险的影响。

研究在以色列开展,收录22岁以上因泌尿感染接受氟喹诺酮治疗患者用药数据,采用多元逻辑回归分析方法评估氟喹诺酮治疗与氟喹诺酮耐药大肠杆菌风险的相关性。

参与人群中,共300105例大肠杆菌感染者,1899168例为非感染者,其中270190名女性参与者中,存在45427例(16.8%)耐药病例,29915名男性参与者中,8835例耐药感染(29.5%)研究发现,抗生素使用量较大地区,健康人群中检测到氟喹诺酮耐药大肠杆菌的风险增加,以药物剂量分层,女性使用量最高1/5人群,相对于次高1/5、中间1/5次低1/5以及最低1/5人群的耐药菌阳性风险比值比分别为1.15、1.31、1.41以及1.51;男性人群则为1.17、1.24、1.35以及1.50,该研究结果在调整抗生素使用史以及其他抗生素类型后仍保持显著。
