Thyroid:放射性碘疗法对女性甲状腺癌患者卵巢功能和生育能力的影响

尽管荟萃分析显示RA​I治疗后AMH水平显著下降,但大多数研究表明DTC患者RAI治疗与长期妊娠率无关。

甲状腺癌是青少年和年轻人中最常见的癌症之一,在过去的三十年中发病率迅速上升。手术是分化型甲状腺癌(DTC)患者的标准治疗方法,必要时进行放射性碘(RAI)治疗。近日,甲状腺疾病领域权威杂志Thyroid上发表了一篇研究文章,这项研究旨在评估RAI治疗对女性卵巢功能和生育能力可能的影响。



研究人员系统地检索PubMed、Embase和Web of Science数据库,截至2020年1月。此外,研究人员对RAI后抗穆勒激素(AMH)水平,RAI之前和之后的AMH水平,以及接受RAI和未接受RAI的甲状腺癌患者妊娠率的试验进行了荟萃分析。

该荟萃分析共有36篇文献符合筛选的条件,其中包括22项研究。大多数研究为回顾性设计。RAI后的第一年出现月经不调的情况为12%,最高为31%。RAI后第一年内,约有8%-16%的患者经历了闭经。与未接受RAI治疗的女性相比,接受RAI治疗的妇女(中位剂量为3700 MBq(范围1110-40700 MBq)更年期年龄略小,分别为51岁和49.5岁(P<0.001)。7项研究报告RAI后AMH浓度为1.79 ng/ml。其中,有四项研究报告了RAI之前和之后1年的AMH浓度,平均差异为1.50 ng/ml。最后,荟萃分析表明接受RAI的患者妊娠风险没有降低。

尽管荟萃分析显示RA​I治疗后AMH水平显著下降,但大多数研究表明DTC患者RAI治疗与长期妊娠率无关。需要进行前瞻性研究来证实这些结果。
