BMJ:怀孕期间使用大环内酯类抗生素是否增加重大出生缺陷风险?

妊娠期使用大环内酯类抗生素与主要出生缺陷风险增加无关。该研究为临床医生、患者和药物监管部门提供了相关科学依据。

大环内酯类抗生素用于治疗常见的细菌感染,如上、下呼吸道感染和性传播疾病,并经常被用作对青霉素过敏的病人的替代品。在美国和欧洲国家,大环内酯类药物(如阿奇霉素、克拉霉素和红霉素)是孕期最常用的抗生素处方。

但孕期使用大环内酯类抗生素是否会增加出生缺陷的风险,以往的研究证据不足。因此,来自丹麦的学者利用丹麦全国性的队列研究,研究了与使用青霉素相比,与未使用抗生素相比,妊娠期使用大环内酯类药物是否会增加重大出生缺陷的风险。研究结果近日发表于医学领域权威期刊BMJ上。

研究者对1997年1月1日至2016年12月31日丹麦所有有记录的怀孕进行了回顾性的队列研究。在1 192 539例活产妊娠中,研究者将使用过大环内酯类药物的妊娠(13 019例)与使用过青霉素的妊娠(在倾向性评分上以1:1的比例匹配),怀孕前不久使用过大环内酯类药物的妊娠(匹配比例为1:1),未使用过抗生素的妊娠(匹配比例为1:4)进行比较。

研究发现,怀孕期间使用过大环内酯类药物的妇女有457名婴儿出生时有重大出生缺陷(每1000次怀孕35.1名),而使用过青霉素的妇女有481名婴儿(每1000次怀孕37.0名)(相对风险比0.95;95%置信区间0.84~1.08),对应的绝对风险差为每1000次怀孕-1.8(95%置信区间-6.4~2.7)。所以与使用青霉素相比,孕期使用大环内酯类抗生素没有增加重大出生缺陷的风险。

与怀孕前近期使用过大环内酯类药物的妇女相比,怀孕期间使用过大环内酯类药物的妇女发生重大出生缺陷的风险没有显著增加(相对风险比1.00(95%置信区间0.88~1.14);每1000例妊娠的绝对风险差-0.1(95%置信区间-4.8~4.7))。

与未使用任何抗生素的妇女相比,孕期使用过大环内酯类药物发生重大出生缺陷的风险也没有显著增加((每1000例妊娠的相对风险比1.05(0.95~1.17);1.8(-1.7~5.3))。

此外与三个比较组相比,也没有发现使用大环内酯类药物会增加12个特定出生缺陷亚组的风险
